牵引钉颌间牵引联合可吸收微型聚合物内固定治疗颌骨骨折的效果

摘　　要：目的探讨在牵引钉颌间牵引基础上应用可吸收微型聚合物内固定治疗颌骨骨折的临床效果。方法选取2021年5月至2022年8月郑州大学第一附属医院收治的280例颌骨骨折患者,以随机数字表法将其分为联合组和对照组,每组140例。对照组接受牵引钉颌间牵引联合钛板内固定术治疗,联合组接受牵引钉颌间牵引联合可吸收微型聚合物内固定治疗。评估两组术后3个月时疗效,比较两组术前及术后6个月时口腔功能、牙周指数,统计两组术后6个月内并发症发生情况。结果术后3个月时,联合组优良率高于对照组(P<0.05);两组术后6个月颞下颌关节功能、张口度以及咬合关系评分较术前低,且联合组低于对照组(P<0.05);两组菌斑指数(PI)、软垢指数(DI)及牙龈指数(GI)水平均高于术前,而联合组低于对照组(P<0.05);联合组并发症总发生率低于对照组(P<0.05)。结论牵引钉颌间牵引联合可吸收微型聚合物内固定更有利于加强牵引钉颌间牵引治疗颌骨骨折的临床效果,可提高患者颞下颌关节功能的改善效果,降低对牙周组织的影响,治疗安全性较好。Objective To investigate the efficacy of intermaxillary traction with traction screws combined with absorbable micropolymer internal fixation in the treatment of jaw fracture.Methods A total of 280 patients with jaw fractures who were admitted to the First Affiliated Hospital of Zhengzhou University from May 2021 to August 2022 were selected,and randomly divided into the combination group and the control group by random number table method,with 140 patients in each group.The control group was treated with intermaxillary traction using traction screws combined with titanium plate internal fixation,while the combination group was treated with intermaxillary traction using traction screws combined with absorbable micropolymer internal fixation.Efficacy in the two groups at 3 months after surgery was evaluated.The two groups were compared in terms of oral function and periodontal index before surgery and at 6 months after surgery.The incidence of complications within 6 months after surgery was calculated.Results Three months after surgery,the excellent and good rate in the combination group was higher than that in the control group(P<0.05).The scores of temporomandibular joint function,mouth opening and occlusal relationship in the two groups 6 months after operation were lower than those before operation,and scores in the combination group were lower than those in the control group(P<0.05).Plaque index(PI),debris index(DI)and gingival index(GI)in the two groups increased,but the three indexes in the combination group were lower than those in the control group(P<0.05).The total incidence of complications in the combination group was lower than that in the control group(P<0.05).Conclusion Intermaxillary traction with traction screws combined with absorbable micropolymer internal fixation is effective in the treatment jaw fracture.The combined treatment can improve patients’temporomandibular joint function,and reduce the impact on periodontal tissues,with good safety.
