黄连温胆汤联合靳三针治疗胆汁反流性胃炎对胃肠激素及血清环氧化酶 核因子-κB的影响

摘　　要：目的探讨黄连温胆汤联合靳三针治疗胆汁反流性胃炎对胃肠激素及血清环氧化酶(COX-2)、核因子-κB(NF-κB)的影响。方法共纳入2022年6月至2023年6月就诊于河南省三门峡市中医院的112例胆汁反流性胃炎患者,按随机数字表法分为对照组(56例,采用铝碳酸镁咀嚼片治疗)和研究组(56例,在对照组治疗方案基础上采用黄连温胆汤联合靳三针治疗),2组患者均进行为期1个月的治疗。比较2组患者治疗前后中医证候积分,胃肠激素指标及血清COX-2、NF-κB的变化,随访2组患者半年,比较复发情况。结果治疗后2组患者胃脘痛、嗳气等中医证候积分与治疗前比较均下降,研究组较对照组低(均P<0.05);治疗后2组患者胃肠激素相关指标水平与治疗前相比均上升,研究组较对照组高(均P<0.05);治疗后2组患者血清COX-2、NF-κB较治疗前下降,研究组较对照组低(均P<0.05);对照组患者有2例复发,复发率为3.6%,研究组患者未见复发,2组复发率比较差异无统计学意义(P>0.05)。结论胆汁反流性胃炎患者应用黄连温胆汤联合靳三针治疗,可有效缓解临床症状,对胃肠激素起到调节作用,并可减轻胃内炎症反应。Objective To explore the effects of Huanglian Wendan Decoction combined with Jin′s Three-Needle Therapy on gastrointestinal hormones and serum cyclooxygenase(COX-2)and nuclear factor-κB(NF-κB)in patients with bile reflux gastritis.Methods A total of 112 patients with bile reflux gastritis who attended San-menxia Hospital of Traditional Chinese Medicine in Henan Province from June 2022 to June 2023.All the pa-tients were divided into a control group(56 cases,treated with hydrotalcite chewable tablets)and a study group(56 cases,treated with Huanglian Wendan Decoction combined with Jin′s Three-Needle Therapy on the basis of the control group′s treatment plan)according to random number table method.Both groups received a one-month treatment.The traditional Chinese medicine syndrome scores,gastrointestinal hormone levels,and serum levels of COX-2 and NF-κB were compared between the two groups before and after treatment.The patients in both groups were followed up for six months to compare the recurrence rates.Results After treatment,the syndrome scores of epigastric pain and belching of the two groups were both decreased compared with before treatment,and the scores of the study group were lower than those of the control group(both P<0.05);the levels of gastrointestinal hormone-related indicators of the two groups were both increased compared with before treatment,and the levels of the study group were higher than those of the control group(both P<0.05);the levels of serum COX-2 and NF-κB of the two groups were both decreased compared with before treatment,and the levels of the study group were low-er than those of the control group(both P<0.05).There were 2 cases of recurrence in the control group,with a re-currence rate of 3.6%,and no recurrence in the study group,and there was no statistically significant difference in the recurrence rate between the two groups(P>0.05).Conclusion The application of Huanglian Wendan Decoc-tion combined with Jin′s Three-Needle Therapy in patients with bile reflux gastriti
