摘　　要：静脉铁剂临床常用于治疗缺铁性贫血,且不能接受口服铁剂或口服铁剂不耐受或需要快速补铁的患者。常用的静脉铁剂包括蔗糖铁、右旋糖酐铁、羧基麦芽糖铁、多聚糖超顺磁氧化铁及异麦芽糖酐铁。近年来出现了输注静脉铁剂后发生低磷血症的大量病例报告。随着更多静脉铁剂相关低磷血症临床研究结果的发表,多国药监部门对输注某铁剂导致低磷血症的不良反应进行相关警示。本文旨在对不同静脉铁剂进行对比,详细阐述静脉铁剂相关低磷血症的机制及临床研究进展,并对文献提及静脉铁剂相关低磷血症的治疗进行汇总,以供临床使用相关药物时予以重视和借鉴。Intravenous iron is commonly used clinically to treat iron deficiency anemia in those who cannot receive oral iron,are intolerant of oral iron,or need rapid iron supplementation.Commonly used intravenous iron formulations include iron sucrose,iron dextran,ferric carboxymaltose,ferumoxytol,and ferric derisomaltose.In recent years,there have been numerous case reports of hypophosphatemia following intravenous iron infusion.As more clinical studies of hypophosphatemia associated with intravenous iron have been published,the drug authorities in several countries have issued warnings about hypophosphatemia associated with the infusion of a certain iron formulation.Therefore,we aim to compare different intravenous iron preparations and review the mechanism of hypophosphatemia induced by intravenous iron and the progress in relevant clinical studies.Finally,the treatment of intravenous iron-related hypophosphatemia mentioned in the literature is summarized to arouse clinical attention and provide references when using related drugs.
