柴胡降脂汤治疗非酒精性脂肪性肝病32例临床观察

摘　　要：目的:探讨柴胡降脂汤治疗肝郁脾虚夹瘀证非酒精性脂肪性肝病(NAFLD)的临床疗效。方法:选取肝郁脾虚夹瘀证NAFLD患者64例,随机数字表法分为治疗组和对照组,每组各32例。对照组用复方甘草酸苷胶囊治疗;治疗组予中药柴胡降脂汤治疗,疗程为12周,比较两组的总体疗效、中医症状评分、肝功能指标(ALT、AST)及血脂指标(TC、TG、LDL-C)水平、肝脏受控衰减参数(CAP)值,并观察两组治疗期间有无不良反应。结果:与治疗前比较,两组治疗后总体疗效、中医症状评分、肝功能指标(ALT、AST)及血脂指标(TC、TG、LDL-C)水平、CAP值均明显降低(P<0.05);治疗组治疗后总体疗效明显高于对照组患者(P<0.05);治疗组治疗后中医症状评分、肝功能指标(ALT、AST)及血脂指标(TC、TG、LDL-C)水平、CAP值较对照组明显降低(P<0.05)。结论:柴胡降脂汤能减轻肝郁脾虚夹瘀证NAFLD的临床症状、恢复肝功能、降低血脂及CAP值,总体疗效显著,且无不良反应,值得临床辨证应用。Objective To investigate the clinical effect of Chaihu Jiangzhi Decoction in treating non-alcoholic fatty liver disease(NAFLD)with liver-stagnation syndrome of spleen-deficiency.Methods 64 cases of NAFLD patients with liver-stagnation and spleen-deficiency syndrome were selected and divided into treatment group and control group by random number table,32 cases in each group.The control group was treated with compound glycyrrhizin capsule,and the treatment group was treated with Chaihu Jiangzhi Decoction reducing decoction for 12 weeks.The overall efficacy,TCM symptom score,liver function index(ALT,AST),blood lipid index(TC,TG,LDL-C)levels,and liver controlled attenuation parameter(CAP)values of the two groups were compared.The two groups were observed for adverse reactions during treatment.Results Compared with before treatment,the overall efficacy,TCM symptom score,liver function index(ALT,AST),blood lipid index(TC,TG,LDL-C)levels and CAP values were significantly decreased in 2 groups after treatment(P<0.05).The overall curative effect of the treatment group was significantly higher than that of the control group(P<0.05).After treatment,the TCM symptom score,liver function index(ALT,AST),blood lipid index(TC,TG,LDL-C)and CAP values in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion Chaihu Jiangzhi Decoction can relieve the clinical symptoms of NAFLD syndrome of liver-stagnation and spleen-deficiency,restore liver function,reduce blood lipids and CAP value,and the overall effect is significant without adverse reactions,which is worthy of clinical differentiation.
