利多卡因用于无痛超声胃镜检查患者的麻醉效果

摘　　要：目的 观察静脉注射利多卡因用于无痛超声胃镜检查患者的麻醉效果。方法 95例行无痛超声胃镜检查的患者分为两组:L组(49例)静脉注射利多卡因1.0mg/kg,随后以1.0mg·kg^(-1)·h^(-1)持续静脉泵注;C组(46例)静脉注射及泵注等体积的生理盐水作为对照。其后,两组均静脉注射舒芬太尼0.1μg/kg和环泊酚0.3~0.4mg/kg。记录两组麻醉诱导前(T0)以及胃镜过咽喉部(T1)、胃镜检查15min(T2)、胃镜检查结束(T3)和患者苏醒时(T4)时的HR、MAP和SpO_(2)。记录环泊酚用量、患者苏醒时间、离室时间及不良反应发生情况。结果 L组术中环泊酚用量及苏醒时间均少于C组(P<0.05)。与C组比较,L组T1时MAP以及T1、T2时SpO_(2)增加(P<0.05);两组各时间点HR比较差异均无统计学意义(P>0.05)。L组低血压、低氧血症及总不良反应发生率低于C组(P<0.05)。结论 静脉注射利多卡因能减少无痛超声胃镜中环泊酚的用量,增加围手术期循环及呼吸稳定性,并减少不良反应的发生。Objective To observe the anesthetic efficacy of intravenous injection of lidocaine in the patients undergoing painless ultrasonic gastroscopy.Methods Ninety patients undergoing painless ultrasonic gastroscopy were divided into two groups.Group L(49 cases)was intravenously injected with lidocaine 1.O mg/kg,which was followed by lidocaine 1.O mg·kg^(-1)·h^(-1) intravenous infusion.Equal volume of normal saline was used in group C(46 cases)as the control.Then sufentanil 0.1μg/kg and ciprofol 0.3~0.4 mg/kg were intravenously given in both groups,respectively.The consumption of ciprofol,awakening time,leaving operation room time and the adverse reactions were recorded.HR,MAP and SpOz were recorded before anesthesia induction(TO),at the time of gastroscope probe passing through throat(T1),15 minutes after the beginning of gastroscopy(T2),at the end of gastroscopy(T3)and awakening(T4).Results The consumption of ciprofol and awakening time in group L were less than those in group C(P<0.05).Compared with group C,MAP at T1 and SpO_(2) at T1 and T2 were increased in group L(P<0.05).There was no statistical difference in HR at T0-T4 between groups of L and C(P>0.05).The incidences of hypotension,hypoxemia and total adverse reactions in group L were lower than those in group C(P<o.05).Conclusion Intravenous injection of lidocaine can reduce the consumption of ciprofol in painless ultrasonic gastroscopy,increase the stability of perioperative circulation and respiration,and reduce the incidence of adverse reactions.
