摘　　要：目的:探讨血清γ-谷氨酰转肽酶、半乳糖凝集素-3在类风湿性关节炎合并心力衰竭患者中的临床意义。方法:选择2019年1月至2023年12月来我院诊治的类风湿性关节炎患者300例,选择来我院体检的健康人群150例作为对照组。根据患者是否合并心力衰竭,将300例患者分为心衰组与非心衰组,类风湿性关节炎患者于入院次日清晨,对照组于体检当日清晨空腹抽血检测血清γ-谷氨酰转肽酶、半乳糖凝集素-3水平,对比心衰组、非心衰组、对照组三组受试者、心衰组Ⅱ、Ⅲ、Ⅳ级患者、心衰组预后良好与预后不良患者的血清γ-谷氨酰转肽酶、半乳糖凝集素-3水平,使用ROC曲线分析血清γ-谷氨酰转肽酶、半乳糖凝集素-3及二者联合对类风湿性关节炎合并心力衰竭诊断及预后价值。结果:血清γ-谷氨酰转肽酶、半乳糖凝集素-3水平:心衰组>非心衰组>对照组,组间两两对比有统计学意义(P<0.05)。心衰组98例患者中,Ⅱ级者41例、Ⅲ级者39例、Ⅳ级者18例。血清γ-谷氨酰转肽酶、半乳糖凝集素-3水平:Ⅳ级>Ⅲ级>Ⅱ级,组间两两对比有统计学意义(P<0.05)。98例心衰患者中,预后良好者76例,预后不良者22例。预后良好者的血清γ-谷氨酰转肽酶、半乳糖凝集素-3水平明显较预后不良者低(P<0.05)。血清γ-谷氨酰转肽酶、半乳糖凝集素-3、二者联合均可用于类风湿性关节炎合并心力衰竭的疾病诊断及预后判断,二者联合较单一指标诊断价值高(P<0.05)。结论:血清γ-谷氨酰转肽酶联合半乳糖凝集素-3可用于类风湿性关节炎合并心力衰竭的疾病诊断及预后判断,二者与心衰严重程度密切相关。Objective:To investigate the clinical significance of serumγ-glutamyltranspeptidase and galactolectin 3 in patients with rheumatoid arthritis complicated with heart failure.Methods:300 patients with rheumatoid arthritis who came to our hospital from January 2019 to December 2023 were selected,and 150 healthy people who came to our hospital for physical examination were selected as the control group.According to whether the patients were combined with heart failure,300 patients were divided into the heart failure group and the non-heart failure group.The rheumatoid arthritis patients were admitted to the hospital in the morning of the next day,and the control group were tested for serumγ-glutamyltranspeptidase and galactin-3 levels by fasting blood sampling in the morning of the physical examination day.The levels of serumγ-glutamyltranspeptidyase and galactin-3 in patients with heart failure group,non-heart failure group and control group,patients with heart failure groupⅡ,Ⅲ,Ⅳ,and patients with good prognosis and poor prognosis were compared.ROC curve was used to analyze the diagnostic and prognostic value of serumγ-glutamyltranspeptidase,galactin-3 and their combination in rheumatoid arthritis complicated with heart failure.Results:The levels of serumγ-glutamyltranspeptidase and galactin-3 were higher in the heart failure group than in the non-heart failure group than in the control group(P<0.05).Among the 98 patients with heart failure,41 cases had grade II,39 cases had grade III,and 18 cases had grade IV.Serum levels ofγ-glutamyltranspeptidase and galactin-3 were as follows:GradeⅣ>gradeⅢ>gradeⅡ,and pair-to-pair comparison between groups had statistical significance(P<0.05).Of 98 patients with heart failure,76 had good prognosis and 22 had poor prognosis.Serum levels ofγ-glutamyltranspeptidase and galactin-3 in patients with good prognosis were significantly lower than those in patients with poor prognosis(P<0.05).Serumγ-glutamyltranspeptidase,galactin-3,and their combination can be used in
