袢钢板重建喙锁韧带治疗肩锁关节脱位的疗效分析

摘　　要：目的 探讨袢钢板重建喙锁韧带治疗肩锁关节脱位患者的疗效分析。方法 回顾性选取52例肩锁关节脱位患者,按照不同手术方式分为对照组(锁骨钩钢板固定治疗)26例和研究组(袢钢板固定治疗)26例。比较两组肩关节活动度、肩锁关节间隙距离和喙锁间距、肩关节功能和疼痛、并发症情况、术后优良率。结果 与术前比较,术后6个月两组肩关节活动度(内旋、外旋、外展)增大,且研究组大于对照组(P<0.05)。与术前比较,术后两组肩锁关节间隙距离和喙锁间距减小,且术后1年研究组小于对照组(P<0.05)。与术前比较,术后两组CMS评分增加,NRS评分减少;且术后6个月、术后1年研究组CMS评分大于对照组,术后6个月研究组NRS评分低于对照组(P<0.05)。研究组并发症发生率低于对照组(P<0.05)。两组优良率比较差异无显著性(P<0.05)。结论 袢钢板重建喙锁韧带手术有利于改善肩锁关节脱位患者的肩关节稳定性,可提高肩关节活动度,促进肩关节功能恢复,降低并发症风险。Aim To explore the therapeutic effect of reconstructing the coracoclavicular ligament with a loop steel plate for the treatment of acromioclavicular joint dislocation.Methods 52 patients with acromioclavicular dislocation were retrospectively selected and divided into control group(clavicular hook plate fixation treatment) with 26 cases and study group(loop plate fixation treatment) with 26 cases according to different surgical methods.The range of motion of shoulder joint,distance of acromioclavicular joint space and coracoclavicular joint,shoulder joint function and pain,complications,postoperative good and good rate,were compared between the two groups.Results Compared with the sitution pre-operation,shoulder joint motion(internal rotation,external rotation and abduction) of both groups increased 6 months after operation,and the study group was greater than the control group(P<0.05).Compared with the sitution pre-operation,the distance of acromioclavicular joint space and coracoclavicular space between the two groups decreased after surgery,and the study group was smaller than the control group one year after surgery(P<0.05).Compared with the sitution pre-operation,CMS score increased and NRS score decreased in both groups after operation.At 6 months and 1 year after surgery,the CMS score of the study group was higher than that of the control group,and the NRS score of the study group was lower than that of the control group at 6 months after surgery(P<0.05).The complication rate of the study group was lower than that of the control group(P<0.05).There was no significant difference in the excellent rate between the two groups(P<0.05).Conclusion The operation of using a loop steel plate to reconstruct the coracoclavicular ligament has the benefit to improve acromioclavicular stability in patients with acromioclavicular dislocation,and can increase shoulder joint rang of motion,promote the recovery of shoulder joint function,and reduce complication risk.
