摘　　要：目的 探讨无症状心律失常官兵高强度军事训练后心肌标志物、心脏功能及心律变化。方法 2019年12月至2021年12月前瞻性纳入73例无症状心律失常官兵(无症状心律失常组),同时选择75名健康官兵作为健康对照组。在同等高强度军事训练前后测量其心肌标志物水平,同时进行心脏彩色多普勒超声检查及24 h动态心电图检查,分析训练前后相关指标变化。结果 两组训练后肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平均较训练前升高,且差异均有统计学意义(P均<0.001),但并未达到病理预警值。两组训练后左心室射血分数(LVEF)较训练前均略有降低,但差异均无统计学意义(P均>0.05)。训练后无症状心律失常组心率、最快心率、室上性早搏次数均高于健康对照组,且差异均有统计学意义(P均<0.05)。结论 无症状心律失常官失在高强度军事训练后CK、CK-MB水平及室上性早搏次数增加,LVEF略有降低,发生恶性心律失常的风险增加。Objective To explore the changes of cardiac function,myocardial markers and arrhythmia in soldiers with asymptomatic arrhythmia after high-intensity military training.Methods Totally 73 asymptomatic arrhythmia soldiers and 75 healthy soldiers were included.Before and after high-intensity military training,their myocardial markers,cardiac ultrasound,and dynamic electrocardiogram were measured,and the changes in relevant indicators before and after the training were analyzed.Results After high-intensity training,CK isoenzyme,and creatine kinase increased compared to before training(all P<0.001),but did not reach the pathological warning value.The case group and control group showed a slight decrease in ejection fraction after training,but the clinical changes were not significant(both P>0.05).In 24-hour dynamic electrocardiogram,the total number of heart beats,average heart rate,and number of supraventricular premature beats significantly increased after high-intensity training(all P<0.05).Conclusion High intensity military training can increase the frequency of supraventricular premature beats in asymptomatic arrhythmia patients.
