鼻内镜联合局部应用曲安奈德鼻喷雾剂治疗鼻窦炎患者疗效及复发率的观察

摘　　要：目的分析鼻内镜联合局部应用曲安奈德鼻喷雾剂治疗鼻窦炎患者疗效及复发率。方法对我院收治的鼻窦炎患者进行研究。随机分为两组(参照组与实验组)并给予不同治疗方案(参照组实施鼻内镜手术,实验组在此基础上增加曲安奈德鼻喷雾剂治疗)。观察两组患者治疗前后粘膜形态及嗅觉功能、治疗疗效及复发情况。结果治疗前,两组粘膜形态及嗅觉功能差异无意义,P>0.05;治疗后,实验组粘膜形态(2.12±0.53)分、嗅觉功能(0.72±0.13)分,均更低于参照组的(3.84±1.02)分、(1.14±0.22)分,P<0.05。实验组治疗有效率95.92%,高于参照组的69.39%,P<0.05。实验组鼻塞发生率6.12%、脓涕发生率4.08%、头痛发生率10.20%、半年复发率0.00%、1年复发率2.04%,均低于参照组的20.41%、20.41%、28.57%、10.20%、20.41%,P<0.05。结论在鼻窦炎患者中,鼻内镜联合局部应用曲安奈德鼻喷雾剂治疗能有效改善患者临床症状,促进鼻腔功能恢复,提高临床治疗有效性,且术后复发率更低,值得推广应用。Objective To analyze the efficacy and recurrence rate of patients with sinusitis treated by nasal endoscopy combined with local triamcinolone nasal spray.Methods The sinusitis patients treated in our hospital were studied.They were randomly divided into two groups(the control group and the experimental group)and given different treatment plans(the control group received nasal endoscopic surgery,and the experimental group was additionally treated with triamcinolone nasal spray).Mucosa morphology,olfactory function,therapeutic effect and recurrence were observed in 2 groups before and after treatment.Results Before treatment,there was no significant difference in mucosa morphology and olfactory function between the two groups(P>0.05).After treatment,mucosa morphology(2.12±0.53)points and olfactory function(0.72±0.13)points in the experimental group were lower than those in the control group(3.84±1.02)points and(1.14±0.22)points,P<0.05.The effective rate of the experimental group was 95.92%,which was higher than that of the reference group(69.39%),P<0.05.The incidence of nasal congestion in the experimental group was 6.12%,4.08%,10.20%,0.00%and 2.04%in six months and one year,all lower than 20.41%,20.41%,28.57%,10.20%and 20.41%in the control group(P<0.05).Conclusions In patients with sinusitis,nasal endoscopy combined with local application of triamcinolone nasal spray can effectively improve the clinical symptoms of patients,promote the recovery of nasal function,improve the effectiveness of clinical treatment,and lower postoperative recurrence rate,which is worthy of promotion and application.
