多层螺旋CT与数字X线摄影在儿童副鼻窦炎诊断中的准确性及临床价值比较研究

摘  要:目的探究多层螺旋CT与数字X线摄影在儿童副鼻窦炎诊断中的准确性及临床价值。方法选取116例2021年2月至2023年2月本院收治疑似副鼻窦炎儿童进行临床资料回顾性分析。入组儿童入院后均接受多层螺旋CT检查、数X线摄影检查,疾病经病理检查确诊。统计检查结果,对比多层螺旋CT与数字X线摄影的灵敏度、特异度、准确度,观察两种检查方法下儿童副鼻窦炎的影像表现。结果116例患儿经病理检查确诊副鼻突炎110例;多层螺旋CT与数字X线摄影的阳性预测值99.09%、96.15%,阴性预测值83.33%、16.67%,灵敏度99.09%、90.91%,特异度83.33%、33.33%,准确度98.28%、87.93%:灵敏度、准确度多层螺旋CT明显高于数字X线摄影(P<0.05)。多层螺旋CT与数字X线摄影影像中多见窦壁骨质硬化、鼻实内软组织密度影、窦腔积液、窦腔粘膜增厚、窦腔透光度减低、窦腔内气液面等表现。结论儿童副鼻窦炎诊断中多层螺旋CT与数字X线摄影灵敏度、准确度较高,相对而言多层螺旋CT诊断效能更高,利于疾病检出与类型鉴别,可为临床医师提供准确检查依据。Objective To explore the accuracy and clinical value of multilayer spiral CT and digital X-ray photography in the diagnosis of accessory sinusitis in children.Methods 116 children admitted from February 2021 to February 2023 were selected for retrospective analysis of clinical data.After admission,the enrolled children underwent multilayer-spiral CT examination and digital X-ray examination,and the disease was confirmed by pathological examination.Statistical examination results,compare the sensitivity,specificity and accuracy of multi-layer spiral CT and digital X-ray photography,and observe the image performance of parananusitis in children under the two examination methods.Results l10 cases of paranasal sinusitis were confirmed in 116 children by pathological examination;positive predictive 99.09%,96.15%,negative predictive 83.33%,16.67%,sensitivity 99.09%,90.91%,specificity 83.33%,33.33%,accuracy 98.28%and 87.93%;sensitivity and accuracy of multi-lice CT were significantly higher than digital X-ray CT(P<0.05).In the multi-layer spiral CT and digital X-ray images,sinus wall bone sclerosis,soft tissue density,sinus lumen effusion,sinus lumen mucosa thickening,reduced sinus lumen transmittance,and air and fluid surface in the sinus cavity.Conclusion In the diagnosis of multi-layer CT and digital X-ray photography in children,the diagnostic efficiency of multi-layer CT is higher,which is conducive to the identification of disease detection and type,and can provide accurate examination basis for clinicians.
