摘 要:目的评估鼻内镜下低温等离子消融术(TCRF)、动力切割术应用在儿童腺样体肥大(AH)治疗中的临床效果。方法纳入2020年2月-2022年12月的100例AH患儿,参照随机数字表法划分A组(纳入50例,行动力切割术)、B组(纳入50例,行TCRF),评价组间治疗有效率、睡眠呼吸障碍儿童生活质量问卷(OSA-18)、症状评分、手术相关参数、术后并发症。结果(1)在治疗有效率方面,A组(100.00%)与B组(100.00%)无差异,P>0.05;(2)术前、术后3个月,组间0SA-18、症状评分均无差异,P>0.05;(3)较之A组,B组手术时间、术中失血量更少,P<0.05;(4)在术后并发症发生率方面,A组(6.00%)与B组(4.00%)无差异,P>0.05。结论TCRF、动力切割术均能有效治疗AH患儿,并且两种术式并发症少,可以尽快改善症状,提高生活质量,不过TCRF在手术时间及减少术中失血量方面更具优势。Objective To evaluate the clinical effect of endoscopic low temperature plasma ablation(TCRF)and dynamic resection in the treatment of adenoid hypertrophy(AH)in children.Method A total of 100 children with AH from February 2020 to December 2022 were included and divided into group A(50 patients were included and underwent dynamotomy)and group B(50 patients were included and underwent TCRF)according to the random number table method.Intergroup response rate,OSA-18,symptom score,parameters related to surgery,and postoperative complications were evaluated.Results(l)There was no difference in treatment effectiveness between group A(100.00%)and group B(100.00%),P>0.05;(2)There were no differences in OSA-18 and symptom scores between groups before and 3 months after surgery,P>0.05;(3)Compared with group A,the operation time and intraoperative blood loss in group B were less,P<0.05;(4)There was no difference in the incidence of postoperative complications between group A(6.00%)and group B(4.00%),P>0.05.Conclusions TCRF and dynamic dissection are effective in the treatment of AH children,and the two procedures have fewer complications,can improve symptoms as soon as possible,improve the quality of life,but TCRF has more advantages in the operation time and reduce intraoperative blood loss.
