基于纯音测听诊断噪声聋的临床研究

摘  要:目的探讨分析基于纯音测听诊断噪声聋方面的实际效果和临床价值,为更加有效准确地开展噪声聋诊断,防治职业性噪声聋提供参考依据。方法选取2021年1月2022年9月期间在我所进行噪声聋诊断分析的212例疑似职业性噪声聋患者为研究对象,对所有研究对象进行三次纯音测听测试,将三次测试结果相比较,计算出三次听阀均值差别、频率差别范围标准差、均值,综合上述研究结果评估基于纯音测听测定在诊断噪声聋方面的实际效果和临床价值。结果以三次纯音听阈测试结果进行对比,通过对比结果可得出,183例研究对象(366耳)连续三次纯音测听结果听阈均值差除个别频率外,其余各频率均介于-0.2至1.4dB。最大标准差于3KHz处为16.9dB,三次听阈测定差值均在土5dB允许误差范围之内。结论纯音测听结果可以作为职业性噪声聋评定和鉴别的参照,整体可靠性和参照性较高,同时纯音测听测定检查方法简单易操作,同时具备快速、无创的优点,必要时可配合其他综合听力测试,进一步实现对职业性噪声聋的客观诊断判定。Objective To explore and analyze the practical effect and clinical value of noise deafness diagnosis based on pure tone audiometry,so as to provide reference for more effective and accurate noise deafness diagnosis and prevention and treatment of occupational noise deafness.Methods A total of 212 patients with suspected occupational noise deafness who were diagnosed and analyzed by me during January 2021 and September 2022 were selected as the research objects.All the subjects were tested by the third pure tone audiometry test.The results of the three tests were compared to calculate the mean difference of the third listening valve,the standard deviation of the range of frequency difference and the mean value.Based on the above results,the actual effect and clinical value of pure tone audiometry in the diagnosis of noise deafness were evaluated.Results The results were compared by the test results of cubic pure tone hearing threshold.It can be seen from the comparison results that the mean difference of listening valve of 212 subjects(424 ears)in continuous cubic pure tone hearing results was between-0.2 and 1.4dB except for some frequencies.The maximum standard deviation at 3KHz was 16.9dB,and the difference values measured by the three listening valves were all within the allowable error range of±5dB.Conclusion The results of pure tone audiometry can be used as the preliminary basis for the diagnosis of occupational noise deafness and the evaluation of the degree of hearing loss,with high reliability.Meanwhile,the test method of pure tone audiometry is simple,easy to operate,and has the advantages of fast and non-invasive.When necessary,it can be combined with other comprehensive hearing tests to further realize the objective diagnosis and determination of occupational noisedeafness.
