摘  要:目的 评估心率(HR)对双源CT大螺距心电门控头颈-主动脉及冠状动脉一站式CT血管成像(CTA)图像质量的影响.方法 连续性选取进行双源CT 一站式扫描的172患者,主观图像质量由2位医师使用4分标准盲评,使用受试者工作特征(ROC)曲线分析HR对图像质量的影响,并基于HR分为高HR组(A组)及低HR组(B组),以感兴趣区(ROI)内CT值、信噪比(SNR)及对比噪声比(CNR)评估客观图像质量.结果 HR≤64次/min冠状动脉图像质量合格,A组主动脉起始部评分低于B组(P<0.05).B组左旋支SNR高于A组但差异无统计学意义(P=0.05),余冠状动脉各分支B组强化程度、SNR、CNR数值均高于A组且差异有统计学意义(P<0.05).B组主动脉起始部、颈动脉腔内强化程度更高且差异有统计学意义(P<0.05).结论 双源CT大螺距心电门控头颈-主动脉及冠状动脉一站式CTA具有可行性,为提高图像质量应常规控制患者HR<64次/min.Objective To evaluate the impact of heart rate(HR)on the image quality of one-stop computed tomography angiogra-phy(CTA)of the head and neck,aorta and coronary artery with dual-source CT ECGgated high-pitch.Methods A total of 172 patients underwent dual-source CT one-stop scan were continuously selected.The subjective image quality was graded on a 4-point scale by two radiologists blinded.Receiver operating characteristic(ROC)curve was performed to analyze the impact of HR on image quality.The patients were divided into high HR group(group A)and low HR group(group B)according to HR.The objective image quality was evaluated by CT value,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)in region of interest(ROI).Results HR<64 beats/min coronary artery image quality were qualified.The aortic root score in group A was lower than that in group B(P<o.05).The left cir-cumflex SNR in group B was higher than that in group A,but the difference was not statistically significant(P=0.05).The enhancement degree,SNR and CNR of all branches of residual coronary artery in group B were higher than those in group A,and the difference was statistically significant(P<0.05).The attenuation of aortic root and carotid artery in group B was higher than that in group A,and the difference was statistically significant(P<0.05).Conclusion One-stop CTA of the head and neck,aorta and coronary artery with dual-source CT ECG-gated high-pitch is feasible.To improve image quality,the patient's HR should be routinely controlled below 64 beats/min.
