摘 要:目的研究肿瘤坏死因子-α(TNF-α)、白细胞介素-1B(IL-1B)、γ-干扰素(IFN-γ)在儿童川崎病合并冠状动脉病变中的价值。方法选取2023年1月-7月石城县人民医院和广州市第一人民医院诊治的50例川崎病儿童作为观察组,并选取同期石城县人民医院体检健康儿童50例作为对照组,均进行TNF-α、IL-1B、IFN-γ水平检测,同时进行超声心动图检查,依据冠脉病变将观察组患儿分为冠损亚组(n=23)和无冠损亚组(n=27),比较观察组和对照组、冠损亚组和无冠损亚组患儿TNF-α、IL-1B及IFN-γ水平,并采用Pearson分析TNF-α、IL-1B及IFN-γ水平与儿童川崎病合并冠状动脉病变的相关性,以及TNF-α、IL-1B及IFN-γ水平对儿童川崎病合并冠状动脉病变的预测价值。结果观察组TNF-α、IL-1B、IFN-γ均高于对照组(P<0.05);冠损亚组TNF-α、IL-1B、IFN-γ均高于无冠损亚组(P<0.05);Pearson相关性分析显示,血清TNF-α、IL-1B、IFN-γ水平与儿童川崎病合并冠状动脉病变呈正相关(P<0.05);TNF-α、IL-1B、IFN-γ的ROC-AUC分别为0.774、0.785、0.761,依据约登指数最大原则,取TNF-α、IL-1B、IFN-γ临界值,此时预测儿童川崎病合并冠状动脉病变的敏感度分别为88.21%、85.12%、87.23%,特异度分别为73.46%、75.00%、73.60%。结论儿童川崎病合并冠状动脉病变后会一定程度影响TNF-α、IL-1B、IFN-γ水平,因此动态监测以上指标水平可一定程度预判是否合并冠状动脉病变,为临床诊治提供有效的理论基础。Objective To study the value of tumor necrosis factor-α(TNF-α),interleukin-1B(IL-1B)and interferon-γ(IFN-γ)in children with Kawasaki’s disease complicated with coronary artery disease.Methods From January to July 2023,50 children with Kawasaki’s disease diagnosed and treated in Shicheng County People's Hospital and Guangzhou First People's Hospital were selected as the observation group,and 50 healthy children who underwent physical examination in Shicheng County People's Hospital during the same period were selected as the control group.The levels of TNF-α,IL-1 B and IFN-γwere detected,and echocardiography was performed at the same time.According to coronary artery lesions,the children in the observation group were divided into coronary artery lesion subgroup(n=23)and non-coronary artery lesion subgroup(n=27).The levels of TNF-α,IL-1 B and IFN-γwere compared between the observation group and the control group,coronary artery lesion subgroup and non-coronary artery lesion subgroup.Pearson was used to analyze the correlation between the levels of TNF-α,IL-1B and IFN-γand coronary artery disease in children with Kawasaki’s disease,and the predictive value of TNF-α,IL-1B and IFN-γlevels for coronary artery disease in children with Kawasaki’s disease.Results The levels of TNF-α,IL-1 B and IFN-γin the observation group were higher than those in the control group(P<0.05).The levels of TNF-α,IL-1 B and IFN-γin the coronary lesion subgroup were higher than those in the non-coronary lesion subgroup(P<0.05).Pearson correlation analysis showed that serum levels of TNF-α,IL-1 B and IFN-γwere positively correlated with coronary artery disease after Kawasaki disease in children(P<0.05).The ROC-AUC of TNF-α,IL-1B and IFN-γwere 0.774,0.785 and 0.761,respectively.According to the principle of maximum Youden index,the critical values of TNF-α,IL-1B and IFN-γwere taken,the sensitivity of predicting coronary artery disease in children with Kawasaki’s disease was 88.21%,85.12%and 87.23%,and the specifi
