缬沙坦联合苯磺酸左旋氨氯地平片在基层卫生院治疗高血压中的疗效观察

摘　　要：目的 分析缬沙坦联合苯磺酸左旋氨氯地平片在基层卫生院治疗高血压中的疗效。方法 选取本基层卫生院在2023年1月至2023年12月诊治的248例高血压患者为研究对象,采用随机数字表法将其分为对照组与试验组,每组124例。对照组给予口服缬沙坦治疗,试验组在对照组的基础上联合苯磺酸左旋氨氯地平片治疗。观察两组治疗前后血压水平、血管内皮功能[一氧化氮(NO)、血管内皮生长因子(VEGF)、内皮素-1(ET-1)、血管舒张功能(FMD)]、血清学指标[C反应蛋白(CRP)、同型半胱氨酸(HCY)、血清肌酐(Scr)、尿素氮(BUN)]、疗效及不良反应。结果 治疗后,试验组收缩压、舒张压以及平均动脉压低于对照组(P <0.05)。治疗后,试验组NO、VEGF以及FMD水平高于对照组,ET-1、CRP、Hcy、BUN以及Scr低于对照组(P <0.05)。试验组治疗总有效率高于对照组(P <0.05)。试验组不良反应发生率与对照组比较无统计学意义(P> 0.05)。结论 在基层卫生院治疗高血压中,缬沙坦联合苯磺酸左旋氨氯地平片不仅更利于降低血压水平,还能改善血管内皮功能,降低机体炎性反应程度,改善肾功能,从而显著提高临床疗效,且不会增加不良反应。Objective To analyze the efficacy of valsartan combined with amlodipine besylate tablets in the treatment of hypertension in primary health clinics.Methods A total of 248 hypertensive patients diagnosed and treated in our primary health center from January 2023 to December 2023 were selected as the study objects.They were randomly divided into a control group and an experimental group using a random number table method,with 124 patients in each group.The control group was given oral valsartan treatment,and the experimental group was combined with levamlodipine besylate tablets on the basis of the control group.Observe the blood pressure levels,endothelial function [nitric oxide(NO),vascular endothelial growth factor(VEGF),endothelin-1(ET-1),vasodilation function(FMD)],serological indicators [C-reactive protein(CRP),homocysteine(HCY),serum creatinine(Scr),urea nitrogen(BUN)],efficacy,and adverse reactions before and after treatment in two groups.Results After treatment,the systolic blood pressure,diastolic blood pressure,and mean arterial pressure in the experimental group were lower than those in the control group(P<0.05).After treatment,the levels of NO,VEGF,and FMD in the experimental group were higher than those in the control group,while ET-1,CRP,Hcy,BUN,and Scr were lower than those in the control group(P<0.05).The total effective rate of the experimental group,which was higher than the control group(P<0.05).The incidence of adverse reactions in the experimental group which was not significantly different from the in the control group(P>0.05).Conclusions In the treatment of hypertension in primary health centers,the combination of valsartan and amlodipine besylate tablets is not only more conducive to reducing blood pressure levels,but also improves vascular endothelial function,reduces the degree of inflammatory response in the body,and improves renal function,thereby significantly improving clinical efficacy without increasing adverse reactions.
