摘　　要：目的 分析家庭医生签约团队管理模式在原发性高尿酸血症患者健康管理中的应用效果。方法 选取2020年10月至2023年10月于厦门梧村街道社区卫生服务中心接受治疗的原发性高尿酸血症患者100例,应用摸球法进行分组,对照组、观察组各50例,对照组管理方案为常规门诊就诊治疗、观察组为家庭医生签约团队管理模式,比较两组血尿酸水平、健康行为、自我管理能力。结果 干预前组间血尿酸水平无统计学意义(P> 0.05);干预后,观察组血尿酸水平较对照组更低(P <0.05)。干预前后评价两组健康行为,干预前组间健康促进生活方式量表-Ⅱ(HPLP-Ⅱ)评分无统计学意义(P> 0.05)。干预后,观察组HPLP-Ⅱ评分均较对照组更高(P <0.05)。干预前后评价自我管理能力,干预前组间自我管理能力(ESCA)评分无统计学意义(P> 0.05);干预后,观察组ESCA各维度评分均较对照组更高(P <0.05)。结论 在原发性高尿酸血症患者健康管理中实施家庭医生签约团队管理模式,对于促进患者血尿酸水平降低,提升其健康行为及自我管理能力的价值显著。Objective To analyze the effect of family doctor contract team management model in health management of patients with primary Hyperuricemia disease.Methods A total of 100 patients with primary hyperuricemia who received treatment in Xiamen Wucun Community Health Service Center from October 2020 to October 2023 were selected and divided into groups by touch ball method,with 50 cases in each control group and observation group.The management scheme of the control group was routine outpatient treatment,and the management mode of the observation group was contracted by family doctors.Blood uric acid level,health behavior and self-management ability were compared between the two groups.Results There was no significant difference in blood uric acid level between groups before intervention(P>0.05).After intervention,the serum uric acid level in the observation group was lower than that in the control group(P<0.05).The health behaviors of the two groups were evaluated before and after the intervention,and the scores of the inter-group Health Promotion Lifestyle Scale-Ⅱ(HPLP-Ⅱ) before the intervention had no statistical significance(P>0.05).After intervention,HPLP-Ⅱ scores in the observation group were higher than those in the control group(P<0.05).The selfmanagement ability was evaluated before and after intervention,and the inter-group self-management ability(ESCA) score was not statistically significant before intervention(P>0.05).After intervention,the ESCA scores of the observation group were higher than those of the control group(P<0.05).Conclusions The application of family doctor contract team management model in the health management of patients with primary hyperuricemia is of significant value in promoting the reduction of serum uric acid level and improving the health behavior and self-management ability of patients.
