基于医护一体化的加速康复外科护理在腹股沟斜疝手术患者中应用研究

摘　　要：目的 探讨基于医护一体化的加速康复外科(ERAS)护理在腹股沟斜疝手术患者中的应用效果。方法 采用便利抽样法,选取2020年7月至2023年7月我院收治的200例腹股沟斜疝手术患者,按随机数字表法分两组,每组100例。对照组采用常规护理,观察组采用基于医护一体化的ERAS护理。对比两组不良情绪、生理应激反应、术后恢复情况、并发症发生率。结果 护理后,观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于对照组(P <0.05)。观察组术后24 h的促肾上腺皮质激素(ACTH)、皮质醇(COR)、去甲肾上腺素(NE)、舒张压(DBP)、收缩压(SBP)及术后72 h的视觉模拟评分法(VAS)评分低于对照组(P <0.05)。观察组术后恢复时间短于对照组,并发症少于对照组(P <0.05)。结论 基于医护一体化的ERAS护理可以减轻腹股沟斜疝手术患者不良情绪与生理应激反应,加快术后康复,缓解疼痛程度,并减少并发症。Objective To explore the application effect of ERAS nursing based on medical integration in patients undergoing inguinal hernia surgery.Methods Convenience sampling was used to select 200 patients who underwent inguinal hernia surgery in our hospital from July 2020to July 2023.They were randomly divided into two groups of 100 patients each using a random number table method.The routine nursing care was given to control group,and the ERAS nursing care was given to observation group based on integrated medical care.The two groups' incidence of adverse emotions,physiological stress reactions,postoperative recovery,and complications were compared.Results After nursing,the observation group showed lower scores of Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS) than the control group(P<0.05).The levels of adrenocorticotropic hormone(ACTH),cortisol(COR),norepinephrine(NE),diastolic blood pressure(DBP),and systolic blood pressure(SBP) 24 hours after surgery and the visual analog scale(VAS) score at 72 hours after surgery in the observation group were lower than in the control group,with statistical difference(P<0.05).The observation group had shorter postoperative recovery time and lower incidence of complications compared to the control group(P<0.05).Conclusions ERAS nursing based on integrated medical care can alleviate negative emotions and physiological stress reactions in patients undergoing inguinal hernia surgery,accelerate postoperative recovery,alleviate pain levels,and reduce complications.
