摘  要:目的:探讨早期认知语言康复训练联合康复治疗对脑损伤早产儿临床症状改善、脑功能发育状况及病情转归的影响。方法:选取130例脑损伤早产儿为研究对象,按干预方案不同分为观察组(n=65)及对照组(n=65)。对照组给予常规康复治疗;观察组在对照组的基础上给予早期认知语言康复训练,持续干预12个月。比较两组患儿干预第3、6、9、12个月贝利婴幼儿发展量表(BSID)评分及干预第6个月、12个月的Gesell发育量表评分,比较两组患儿干预前后振幅整合脑电图最高电压、最低电压水平,观察患儿后遗症发生情况。结果:随着时间推移,两组BSID量表智力发育指数(MDI)、心理运动发育指数(PDI)评分均升高(P<0.05),且观察组均高于对照组(P<0.05);随着时间推移,两组Gessell量表大运动、精细运动、语言、适应性、个人社交DQ评分均升高(P<0.05),且观察组均高于对照组(P<0.05);干预第12个月,观察组振幅整合脑电图最高电圧、最低电压均高于对照组(P<0.05);观察组后遗症发生率为12.31%,低于对照组的26.15%(P<0.05)。结论:在常规康复治疗的基础上联合应用早期认知语言康复训练可有效提高脑损伤早产儿智能发育水平,促进其损伤神经系统修复,降低后遗症风险,改善患儿预后。Objective:To investigate the effects of early cognitive and language rehabilitation training combined with rehabilitation therapy o n the improvement of clinical symptoms,brain function development,and prognosis of preterm infants with brain injury.Methods:A total of 130 preterm infants with brain injury were divided into an observation group(n=65)and a control group(n=65)according to different intervention plans.The control group received conventional rehabilitation therapy,while the observation group received early cognitive and language rehabilitation training in addition to conventional therapy for 12 months.The BSID scores at different time points(3,6,9,and 12 months post-intervention),Gesell Developmental Schedules scores at 6 and 12 months post-intervention,pre-and post-intervention amplitude-integrated electroencephalogram maximum and minimum voltage levels,and sequelae status were compared between the two groups.Results:Over time,BSID Mental Development Index(MDI)and Psychomotor Development Index(PDI)scores increased in both groups(P<0.05),with the observation group showing higher scores than the control group(P<0.05).Gesell scale scores for gross motor,fine motor,language,adaptive,and personal-social DQ increased in both groups(P<0.05),with higher scores in the observation group(P<0.05).After 12 months,the observation group had higher maximum and minimum voltages on the amplitude-integrated electroencephalogram than the control group(P<0.05).The incidence of sequelae was lower in the observation group(12.31%)compared to the control group(26.15%)(P<0.05).Conclusion:The combination of early cognitive and language rehabilitation training with conventional rehabilitation therapy can effectively improve the intelligence development level,promote the repair of the damaged nervous system,reduce the risk of sequelae,and improve the prognosis of preterm infants with brain injury.
