冠心病患者心电图QRS波时限、ST-T改变及与疾病严重程度的关系

摘　　要：目的:探讨冠心病(CHD)患者心电图QRS波时限、ST-T改变与疾病严重程度的关系。方法:选取105例CHD患者为研究对象,根据Gensini评分将其分为轻度组(n=30)、中度组(n=47)和重度组(n=28)。对各组行实时动态心电图检查QRS波时限及ST-T改变情况,分析心电图QRS波时限及ST-T改变与CHD疾病严重程度的关系,绘制ROC曲线分析QRS波时限及ST-T改变对CHD重度病变的预测价值。结果:CHD组不同严重程度患者QRS波时限、持续性ST-T改变比例比较:轻度组<中度组<重度组(P<0.05);相关性分析显示,QRS波时限、ST-T改变与CHD疾病严重程度分级正相关(r=0.624、0.599,P<0.001);ROC曲线显示,QRS波时限、ST-T改变预测CHD重度病变的曲线下面积分别为0.830、0.748(P<0.001)。结论:CHD患者心电图QRS波时限、ST-T改变与冠脉病变严重程度具有关联性,对CHD患者病情严重程度有一定评估价值。Objective:To investigate the relationship between QRS duration and ST-T changes on the electrocardiogram and the severity of coronary heart disease(CHD).Methods:A total of 105 patients with CHD were included in this retrospective study.According to Gensini score,they were divided into mild group(n=30),moderate group(n=47)and severe group(n=28).All patients received real-time dynamic electrocardiography.QRS duration and ST-T changes on electrocardiograms were observed.The relationship between QRS duration and ST-T changes on the electrocardiogram and the severity of CHD was analyzed.ROC curves were plotted to analyze the predictive value of QRS duration and ST-T changes for severe CHD.Results:QRS duration and the proportion of persistent ST-T changes increased in order from the mild group,the moderate group to the severe group(P<0.05).Correlation analysis found that QRS duration and ST-T changes were positively correlated with the severity of CHD(r=0.624,0.599,P<0.001).ROC curves indicated that the AUC values of QRS duration and ST-T changes for predicting severe CHD were 0.830 and 0.748(P<0.001).Conclusion:QRS duration and ST-T changes on electrocardiograms of patients with CHD are correlated with the severity of CHD,and have certain value in evaluating the severity of CHD.
