摘 要:目的分析血清白细胞介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)水平在评估大面积烧伤患者难治性创面感染中的应用价值。方法选取本院2022年1月至2023年12月就诊的90例大面积烧伤患者,根据是否发生难治性创面感染将其分为2组,30例难治性创面感染者设为观察组,60例非难治性创面感染者设为对照组。比较2组入院1 d、3 d、5 d血清IL-6、CRP、PCT水平;logistic回归分析难治性创面感染的危险因素;绘制受试者工作曲线(ROC),分析血清IL-6、CRP、PCT对难治性创面感染的预测效能。结果观察组血清IL-6、CRP、PCT水平均较对照组更高(P<0.05)。血清IL-6、CRP、PCT、烧伤面积、术后负压引流是难治性创面感染的危险因素(P<0.05)。血清IL-6、CRP、PCT联合预测难治性创面感染的曲线下面积(AUC)(95%CI)为0.868(0.861,0.942),血清IL-6、CRP、PCT联合预测难治性创面感染的灵敏度(93.82%)较各指标单一检测(76.82%、68.86%、71.56%)更高(P<0.05),联合检测特异度(80.48%)与各指标单一检测(81.34%、78.16%、82.28%)比较,差异无统计学意义(P>0.05)。结论大面积烧伤患者难治性创面感染会引起血清IL-6、CRP、PCT水平增高,联合检测血清IL-6、CRP、PCT可提高对大面积烧伤患者难治性创面感染的预测价值。Objective To analyze the application value of serum levels of interleukin-6(IL-6),C-reactive protein(CRP),and procalcitonin(PCT)in evaluating refractory wound infections in patients with extensive burns.Methods Ninety patients with extensive burns who visited our hospital from January 2022 to December 2023 were selected and divided into two groups based on the occurrence of refractory wound infections.Thirty patients with refractory wound infections were assigned to the observation group,and 60 patients with non-refractory wound infections were assigned to the control group.The serum levels of IL-6,CRP,and PCT were compared between the two groups at 1 day,3 days,and 5 days of admission.Logistic regression analysis was performed to identify the risk factors for refractory wound infections,and a receiver operating curve(ROC)was drawn to analyze the predictive efficacy of serum IL-6,CRP,and PCT on refractory wound infections.Results The serum levels of IL-6,CRP,and PCT in the observation group were higher than those in the control group(P<0.05).Serum IL-6,CRP,PCT,burn area,and postoperative negative pressure drainage are risk factors for refractory wound infections(P<0.05).The AUC of serum IL-6,CRP,and PCT combined prediction for refractory wound infection is 0.868,and the 95%confidence interval is 0.861,0.942.The sensitivity of serum IL-6,CRP,and PCT combined prediction for refractory wound infection is higher(93.82%)than single detection(76.8%,68.9%,71.6%),respectively(P<0.05),There was no significant difference in the specificity between joint testing(80.5%)and single testing(81.3%,78.2%,82.3%),respectively(P>0.05).Conclusion Refractory wound infection in patients with extensive burns can cause an increase in serum levels of IL-6,CRP,and PCT.Combined detection of serum IL-6,CRP,and PCT can improve the predictive value of refractory wound infection in patients with extensive burns.
