一起更换循环泵导致管道泄漏起火事故的案例分析

摘　　要：文章介绍了一起更换循环泵导致管道泄漏起火事故,通过现场勘查、调查和相关复验,查明事故直接原因为2名操作工人违规作业,在未停机放油的状态下,带压更换垫片,且进口阀门未关闭,造成有机热载体从循环泵更换处喷涌而出闪燃起火。间接原因为该企业安全生产主体责任落实不到位,现场人员管理不规范,应急处置措施不当。This paper introduces an accident of pipeline leakage and f ire caused by the replacement of circulating pump.Through on-site investigation,investigation and related reinspection,it is found that the direct cause of the accident is two operators’illegal operation.In the state of not stopping the oil,the replacement gasket with pressure and the inlet valve is not closed,which causes the organic heat carrier to erupt from the replacement of circulating pump and flash f ire.The indirect reasons are that the main responsibility for safety production of the enterprise is not in place,the on-site personnel management is not standardized,and the emergency response measures are improper.
