健脾益气活血汤治疗糖尿病并骨质疏松的疗效及对25-(OH)D、β-CTX、Pref-1的影响

摘　　要：目的:探讨健脾益气活血汤治疗糖尿病(diabetes mellitus,DM)并骨质疏松(osteporosis,OP)的疗效及对25羟维生素D[25⁃(OH)D]、β-骨胶原交联(β-CTX)、前脂肪因子-1(Pref-1)的影响。方法:选择2021年6月至2022年6月在河北省沧州中西医结合医院治疗的DM合并OP患者206例,应用随机数表法将其分为对照组(常规治疗)及观察组(联合健脾益气活血汤)各103例,对比2组疗效、治疗前后中医证候积分、骨密度(BMD)、血清25-(OH)D、β-CTX、Pref-1水平及不良反应。结果:观察组总有效率高于对照组(P<0.05)。治疗后,2组腰背疼痛、无力/肌肉关节刺痛、口干舌燥积分、β-CTX、Pref-1水平降低(P<0.05),且观察组低于对照组(P<0.05)。2组血清25-(OH)D水平,L 2~4、股骨颈、股骨大转子BMD均升高(P<0.05),且观察组高于对照组(P<0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论:健脾益气活血汤治疗DM并OP可有效改善患者症状和血清25-(OH)D、β-CTX、Pref-1水平,调节骨代谢,增加BMD,提高疗效。Objective:To investigate the efficacy of Jianpi Yiqi Huoxue Decoction in the treatment of diabetes mellitus(DM)complicated with osteoporosis(OP)and its effects on 25-hydroxyvitamin D[25-(OH)D],β-CrossLaps(β-CTX),,and Preadipocyte Factor 1(Pref-1).Methods:A total of 206 patients with DM complicated with OP who were treated at Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2021 to June 2022 were selected and randomly divided into a control group(conventional treatment)and a study group(combined with Jianpi Yiqi Huoxue Decoction),with 101 patients in each group.The efficacy,adverse reactions,traditional Chinese medicine syndrome scores,bone mineral density(BMD),serum 25-(OH)D,β-CTX,and Pref-1 levels before and after treatment were compared between the two groups.Results:The total effective rate in the study group was higher than that in the control group(P<0.05).After treatment,the scores for low back pain,weakness/muscle and joint tingling pain,dry mouth and thirst,as well as the levels ofβ-CTX and Pref-1 decreased in both groups(P<0.05),and the decreases were more significant in the study group compared to the control group(P<0.05).The serum 25-(OH)D levels and BMD at L 2-4,femoral neck,and greater trochanter increased in both groups(P<0.05),and the results were higher in the study group than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Jianpi Yiqi Huoxue Decoction for the treatment of DM complicated with OP can effectively improve patients'symptoms and serum levels of 25-(OH)D,β-CTX,and Pref-1,regulate bone metabolism,increase BMD,and enhance therapeutic efficacy.
