虾刺伤引起皮肤海分枝杆菌感染一例

摘　　要：患者,男,65岁。右手中指皮疹50天,右上肢皮疹伴痒痛20天。组织病理符合感染性肉芽肿。右前臂皮损宏基因组二代测序(mNGS)提示分枝杆菌属阳性。皮损脓性分泌物涂片抗酸染色阳性,菌落DNA PCR测序鉴定菌株为海分枝杆菌。给予口服利福平及乙胺丁醇4个月后皮疹消退,随访1年无复发。A 65-year-old male patient presented with a rash on his right middle finger for 50 days and a rash with itching and pain on his right upper limb for 20 days.Histopathological examination was consistent with an infectious granuloma.Metagenomic next-generation sequencing(mNGS)of the right forearm skin lesion tissue indicated the presence of Mycobacterium species.The stain of skin purulent secretion was positive for acid fast staining,and the strain was identified as Mycobacterium marinum by PCR sequencing.After the treatment of rifampicin and ethambutol,the rashes of the patient subsided after 4 months and there was no recurrence after 1 year follow up.
