血清内源性配体-13、正五聚蛋白3联合血栓调节蛋白对下肢骨折术后深静脉血栓的诊断效能

摘  要:目的探讨血清内源性配体-13(Apelin-13)、正五聚蛋白3(PTX3)联合血栓调节蛋白(TM)对下肢骨折患者术后发生深静脉血栓(DVT)的诊断效能。方法收集2021年1月至2022年5月于南通市中医院住院治疗的98例下肢骨折患者的临床资料,根据术后是否发生DVT分为DVT组(n=46)与非DVT组(n=52)。分析下肢骨折术后DVT患者血清中Apelin-13、PTX3及TM水平的相关性,下肢骨折患者术后发生DVT的危险因素,以及血清Apelin-13、PTX3及TM水平对下肢骨折患者术后发生DVT的诊断效能。结果DVT组患者血清中的Apelin-13、PTX3、TM水平均明显高于非DVT组患者,差异均有统计学意义(P﹤0.01)。Pearson相关分析结果显示,在下肢骨折术后DVT患者的血清中,Apelin-13与PTX3水平呈正相关(r=0.296,P﹤0.05),PTX3与TM水平呈正相关(r=0.310,P﹤0.05),Apelin-13与TM水平呈正相关(r=0.411,P﹤0.05)。多因素分析结果显示,血清Apelin-13、PTX3及TM均是下肢骨折患者术后发生DVT的独立危险因素(P﹤0.05)。Apelin-13、PTX3及TM三者联合检测对下肢骨折患者术后发生DVT的诊断效能优于Apelin-13、PTX3、TM单独检测(P﹤0.05)。结论下肢骨折术后DVT患者血清中Apelin-13、PTX3、TM水平较高,均是下肢骨折患者术后发生DVT的影响因素,三者联合检测对下肢骨折患者术后发生DVT具有较高的诊断价值。Objective To investigate the diagnostic efficacy of serum apelin-13,pentraxin 3(PTX3)combined with thrombomodulin(TM)for deep vein thrombosis(DVT)after lower extremity fracture surgery.Method A total of 98 patients with lower extremity fractures who were hospitalized in Nantong Hospital of Traditional Chinese Medicine from January 2021 to May 2022 were selected and divided into DVT group(n=46)and non-DVT group(n=52)based on whether DVT occurred after surgery.The correlation between serum levels of Apelin-13,PTX3,and TM in patients with DVT after lower extremity fracture surgery,the risk factors for DVT after lower extremity fracture surgery,and the diagnostic efficacy of serum levels of Apelin-13,PTX3,and TM for DVT after lower extremity fracture surgery were analyzed.Result The levels of Apelin-13,PTX3,and TM in the serum of patients in the DVT group were significantly higher than those in the non-DVT group,and the differences were statistically significant(P<0.01).Pearson correlation analysis showed that in the serum of DVT patients after lower extremity fracture surgery,Apelin-13 level was positively correlated with PTX3 level(r=0.296,P<0.05),PTX3 level was positively correlated with TM level(r=0.310,P<0.05),and Apelin-13 level was positively correlated with TM level(r=0.411,P<0.05).The results of multivariate analysis showed that serum Apelin-13,PTX3,and TM were independent risk factors for DVT after lower extremity fracture surgery(P<0.05).The combined detection of Apelin-13,PTX3,and TM has better diagnostic efficacy for DVT after lower extremity fracture surgery than the individual detection of Apelin-13,PTX3,and TM(P<0.05).Conclusion The levels of Apelin-13,PTX3 and TM in the serum of patients with DVT after lower extremity fracture surgery are relatively increased,all of which are influencing factors for the occurrence of DVT after lower extremity fracture surgery.The combined detection of the three has high diagnostic value for the occurrence of DVT in lower extremity fracture patients after surgery.
