髂静脉压迫综合征的中西医治疗进展

摘　　要：髂静脉压迫综合征(IVCS)指右侧髂动脉及腰椎长时间机械性压迫左髂总静脉,造成静脉腔内粘连、内膜增生和纤维化,引起静脉管腔狭窄、闭塞,造成血流动力学发生改变。大部分患者早期无明显临床症状,随着疾病的进展,可出现下肢静脉功能不全、下肢肿胀、皮肤营养性障碍、下肢溃疡等症状,甚至下肢深静脉血栓形成。IVCS依据是否合并下肢深静脉血栓可分为血栓性髂静脉压迫综合征(TIVCS)和非血栓性髂静脉压迫综合征(NIVCS),治疗方式可分为非手术治疗和手术治疗,腔内治疗已成为IVCS的主要手术方式。中医辨证论治同样对疾病的治疗及预后起到重要的作用。本文结合国内外相关文献对IVCS的中西医治疗进展进行综述,以期为临床治疗IVCS提供新思路。Iliac vein compression syndrome refers to a syndrome in which the right iliac artery and lumbar spine mechanically compress the left common iliac vein for a long period of time,resulting in intravenous adhesion,intimal hyperplasia and fibrosis,which leads to venous stenosis and occlusion,resulting in hemodynamic changes.Most patients have no obvious clinical symptoms in the early stage,and with the progression of the disease,lower extremity venous insufficiency,lower extremity swelling,skin nutritional disorders,lower extremity ulcers,and even deep venous thrombosis of lower extremity may occur.According to whether it is combined with deep vein thrombosis of lower extremity,it can be divided into non-thrombotic iliac vein compression syndrome and thrombotic iliac vein compression syndrome,and the treatment can be divided into non-surgical treatment and surgical treatment,endovascular therapy has become the main surgical method for iliac vein compression syndrome.TCM syndrome differentiation and care also play an important role in the treatment and prognosis of the disease.Combined with the relevant literature at home and abroad,this paper discusses the progress of traditional Chinese and western medicine treatment for iliac vein compression syndrome,in order to provide new ideas for clinical treatment of iliac vein compression syndrome.
