生肌玉红膏联合脐带间充质干细胞外泌体治疗糖尿病足创面的临床研究

摘　　要：目的探讨生肌玉红膏联合脐带间充质干细胞来源的外泌体(HUC-MSC-Ex)治疗糖尿病足创面的疗效。方法收集2022年1月至2023年2月就诊于平顶山市中医医院的64例糖尿病足患者的临床资料,根据治疗方法的不同分为观察组和对照组,每组32例。对照组采用凡士林纱布联合HUC-MSC-Ex进行治疗,观察组采用生肌玉红膏联合HUC-MSC-Ex进行治疗。比较两组患者的临床疗效,创面面积,创面症状、体征积分及创面愈合时间。结果治疗28 d后,观察组患者的临床疗效优于对照组患者,差异有统计学意义(P﹤0.05)。治疗28 d后,两组患者的创面面积均较本组治疗前缩小,创面症状、体征积分均较本组治疗前减少,且观察组患者的创面面积小于对照组患者,创面症状、体征积分低于对照组患者,创面愈合时间短于对照组患者,差异均有统计学意义(P﹤0.05)。结论生肌玉红膏联合脐带间充质干细胞外泌体治疗糖尿病足创面的效果明显,能够提高创面愈合率,缩小创面面积,改善创面症状,缩短创面愈合时间,值得临床推广应用。Objective To investigate the clinical effect of Shengjiyuhong ointment combined with human umbilical cord mesenchymal stem cells exosome(HUC-MSC-Ex)on diabetic foot wounds.Method The clinical data of 64 patients with diabetes foot who visited Pingdingshan Traditional Chinese Medicine Hospital from January 2022 to February 2023 were collected and divided into observation group and control group according to different treatment methods,with 32 cases in each group.The control group was treated with vaseline gauze combined with HUC-MSC-Ex,while the observation group was treated with Shengjiyuhong ointment combined with HUC-MSC-Ex.The clinical efficacy,wound area,integral score of symptoms and signs of wounds,and wound healing time between two groups of patients were compared.Result After 28 days of treatment,the clinical effect of the observation group patients was better than the control group patients,and the difference was statistically significant(P<0.05).After 28 days of treatment,the wound area of both groups of patients decreased compared to before treatment in this group,and the integral scores of symptom and sign of wound decreased compared to before treatment in this group.In addition,the observation group had a smaller wound area and lower integral scores of symptom and sign of wound than the control group,and the wound healing time was shorter than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Shengjiyuhong ointment combined with HUC-MSC-Ex has obvious effect in treating diabetes foot wounds.It can improve the wound healing rate,reduce the wound area,improve the wound symptoms,and shorten the wound healing time,which is worthy of clinical promotion and application.
