摘  要:目的探讨腹主动脉瘤(AAA)发生的影响因素及其与抵抗素(RETN)-420C﹥G的相关性。方法选取2011年7月至2021年12月于解放军总医院就诊的155例AAA患者为病例组,采用病例对照研究设计按照年龄、性别配对条件同期纳入310例门诊筛查的非AAA为对照组,分析AAA发生的影响因素及其与RETN-420C﹥G的相关性。结果吸烟、高血压、症状型外周动脉疾病(PAD)、血脂异常均是发生AAA高危险因素(P﹤0.05)。AAA组患者的RETN-420C﹥G基因型分布与对照组比较,差异无统计学意义(P﹥0.05)。大AAA组的GG基因型分布频率高于小AAA组,差异有统计学意义(P﹤0.05)。与CC基因型相比,GG基因型可增加小AAA发展为大AAA的风险(P﹤0.05),且校正吸烟、饮酒、腰臀比(WHR)、高血压、PAD、慢性阻塞性肺疾病(COPD)、脑卒中、血脂异常因素后GG基因型仍可增加小AAA发展为大AAA的风险(P﹤0.05);RETN–420C﹥G隐性模型结果显示,与CG/CC基因型相比,GG可增加AAA发展为大AAA的风险(P﹤0.05)。结论腹主动脉瘤的发生受吸烟、高血压、高脂血症等多种因素影响,腹主动脉瘤的发生与RETN-420C﹥G无明显相关性,但腹主动脉瘤的进展与RETN-420C﹥G有明显的相关性,有待进一步研究证实。Objective To investigate the factors affecting the occurrence of abdominal aortic aneurysm(AAA)and its correlation with resistin(RETN-420C>G).Method A total of 155 AAA patients treated in the First Medical Center,Chinese PLA General Hospital from July 2011 to December 2021 were selected as the case group.A case-control study design was used to include 310 non-AAA outpatient screening patients as the control group according to age and sex matching conditions during the same period.The influencing factors of AAA occurrence and its correlation with TN-420C>G were analyzed.Result Smoking,hypertension,symptomatic peripheral artery disease(PAD),and dyslipidemia were all risk factors for AAA(P<0.05).The distribution of RETN-420C>G genotypes in AAA group patients showed no statistically significant difference compared to the control group(P>0.05).The distribution frequency of GG genotype in the large AAA group was higher than that in the small AAA group,and the difference was statistically significant(P<0.05).Compared with the CC genotype,GG genotype can increase the risk of small AAA developing into large AAA(P<0.05),and after adjusting for smoking,alcohol consumption,waist hip rate(WHR),hypertension,PAD,chronic obstructive pulmonary disease(COPD),stroke,and dyslipidemia factors,GG genotype can still increase the risk of small AAA developing into large AAA(P<0.05).The results of the RETN-420C>G recessive model showed that compared with the CG/CC genotype,GG increased the risk of AAA developing into large AAA(P<0.05).Conclusion The occurrence of abdominal aortic aneurysm is affected by smoking,hypertension,hyperlipidemia and other factors.There is no significant correlation between the occurrence of abdominal aortic aneurysm and TN-420C>G,but there is a significant correlation between the progression of abdominal aortic aneurysm and TN-420C>G,which needs to be confirmed by further studies.
