摘  要:目的探讨计算机断层扫描血管成像(CTA)联合锥形束计算机断层扫描(CBCT)图像融合技术在老年胸主动脉瘤腔内介入治疗中的应用。方法收集2021年1月到2022年1月青海省心脑血管病专科医院收治的行腔内介入治疗的73例老年胸主动脉瘤患者,按照治疗方法不同将其分为观察组(n=37,术中采用CTA联合CBCT图像融合技术引导)和对照组(n=36,术中采用CTA引导)。记录两组患者治疗成功率、手术时间、造影剂用量、辐照时间、总辐射剂量及手术前后肾功能指标变化情况。结果两组老年胸主动脉瘤患者技术成功率均为100%。观察组患者手术时间、造影剂用量、辐照时间、总辐射剂量均低于对照组,差异均有统计学意义(P﹤0.05)。术后1、3个月,两组患者血肌酐、尿素氮比较,差异均无统计学意义(P﹥0.05)。结论CTA联合CBCT图像融合技术在老年胸主动脉瘤腔内介入治疗能进一步降低造影剂用量和辐照剂量,且具有良好的安全性和有效性,值得在临床上推广应用。Objective To investigate the application of computer tomography angiography(CTA)combined with cone beam computer tomography(CBCT)image fusion technique in endovascular intervention therapy for elderly thoracic aortic aneurysms.Method Seventy-three elderly patients with thoracic aortic aneurysm who underwent intracavitary intervention treatment at Qinghai Provincial Cardiovascular and Cerebrovascular Disease Specialized Hospital from January 2021 to January 2022 were collected and divided into observation group(n=37,guided by CTA combined with CBCT image fusion technology during surgery)and control group(n=36,guided by CTA during surgery)according to different treatment methods.The success rate of treatment,surgical time,contrast agent dosage,irradiation time,total radiation dose,and changes in renal function indicators before and after surgery for two groups were compared.Result The technical success rate of both groups of elderly patients with thoracic aortic aneurysm was 100%.The observation group had lower surgical time,contrast agent dosage,irradiation time,and total radiation dose than the control group,the differences were statistically significant(P<0.05).At 1 and 3 months after surgery,there were no statistically significant differences in blood creatinine and urea nitrogen between the two groups of patients(P>0.05).Conclusion CTA combined with CBCT image fusion technique in endovascular intervention therapy for elderly thoracic aortic aneurysms can effectively reduce the amount of contrast agent and irradiation dose,and has a good safety profile,which is worthy of recommendation.
