摘  要:目的:探索构建科学、合理的手术绩效评价模型,为实现手术绩效精细化管理提供参考依据。方法:基于文献分析法、访谈法和德尔菲法,确定手术绩效评价指标;运用层次分析法、熵权法和组合赋权法,确定手术绩效评价指标权重。结果:构建手术绩效评价指标体系,包含手术难度、手术风险、治疗过程和技术先进性4个一级指标、12个二级指标,运用组合赋权法对模型指标进行赋权和实证分析。结论:手术绩效评价模型能够鼓励医疗新技术的开展,科学体现外科手术的复杂性及风险程度;需结合医院实际,不断更新完善指标体系,使之更加符合绩效评价需求。Objective To explore and construct a scientific and reasonable surgical performance evaluation model,and to provide a reference for achieving refined management of surgical performance.Methods The indexes of surgical performance evaluation were determined by literature analysis,interview method and Delphi method.The weights of surgical performance evaluation indicators were determined by analytic hierarchy process,entropy weight method and combination weighting method.Results The final evaluation index system of surgical performance included 4 first-level indexes of surgical difficulty,surgical risk,treatment process and technical advancement,and 12 second-level indexes.The model indexes were weighted and empirically analyzed by the combination weighting method.Conclusion The surgical performance evaluation model can encourage the development of new medical technologies and scientifically reflect the complexity and risk level of surgical procedures.However,it is necessary to continuously update and improve the indicator system based on the actual situation of the hospital to make it more in line with the needs of performance evaluation.
