摘  要:目的探讨可溶性尿激酶型纤溶酶原激活物受体(suPAR)水平在慢性肾病(CKD)大鼠心、脑、肾组织中的表达。方法采用3种方法建立大鼠CKD模型,分别为IgA肾病(IgAN)组(n=8)、局灶性节段性肾小球硬化(FSGS)组(n=8)与IgA肾病伴局灶节段肾小球硬化(IgAN-FSGS)组(n=10),设立对照组(n=6)。检测各组血suPAR水平,应用免疫组化、蛋白质印记法检测尿激酶型纤溶酶原激活物受体(uPAR)与纤溶酶原激活物抑制物1(PAI-1)在肾、心脏及脑组织中的表达情况。结果本研究建模成功大鼠共26只,其中,对照组5只,IgAN组8只,FSGS组5只,IgAN-FSGS组8只。与对照组比较,IgAN组、FSGS组、IgAN-FSGS组的血肌酐均显著升高,差异有统计学意义(P<0.05)。与对照组比较,IgAN组、FSGS组、IgAN-FSGS组的尿蛋白量均升高,其中,IgAN-FSGS组和FSGS组的尿蛋白量明显高于对照组,差异有统计学意义(P<0.05)。与对照组比较,IgAN-FSGS组和FSGS组的血suPAR水平均升高,差异有统计学意义(P<0.05)。4组大鼠肾组织uPAR、PAI-1均主要表达在肾小球,以肾小球毛细血管壁及足细胞分布为主,强弱趋势依次为IgAN-FSGS组>FSGS组>IgAN组>对照组,肾小管及肾间质未见明显表达。4组大鼠心脏组织uPAR、PAI-1表达强弱不等,组间两两比较,差异无统计学意义(P>0.05)。与对照组比较,IgAN组、FSGS组、IgAN-FSGS组大鼠脑组织uPAR表达略增高,PAI-1表达略降低,组间两两比较,差异无统计学意义(P>0.05)。4组大鼠脑组织PAI-1表达情况,组间两两比较,差异无统计学意义(P>0.05)。4组大鼠心脏及脑组织的uPAR、PAI-1表达均较弱。结论uPAR、PAI-1在CKD大鼠肾小球中高表达,但在心脏、脑组织中表达未增高。Objective To investigate the expression of soluble urokinase-type plasminogen activator receptor(suPAR)in the heart,brain and kidney of chronic kidney disease(CKD)rats.Methods Three methods were used to establish the rat CKD model,namely IgA nephropathy(IgAN)group(n=8),focal segmental glomerulosclerosis(FSGS)group(n=8),IgA nephropathy with focal segmental glomerulosclerosis(IgAN-FSGS)group(n=10),and control group(n=6).Serum suPAR levels were detected in each group,and the expressions of urokinase-type plasminogen activator receptor(uPAR)and plasminogen activator inhibitor 1(PAI-1)in kidney,heart and brain tissues were detected by immunohistochemistry and western blotting methods.Results A total of 26 rats were successfully modeled in this study,including 5 in the control group,8 in the IgAN group,5 in the FSGS group,and 8 in the IgAN-FSGS group.Compared with the control group,the serum creatinine of IgAN group,FSGS group and IgAN-FSGS group was significantly increased,and the difference was statistically significant(P<0.05).Compared with the control group,the urinary protein of IgAN group,FSGS group and IgAN-FSGS group was increased,and the urinary protein of IgAN-FSGS group and FSGS group was significantly higher than that of control group,the difference was statistically significant(P<0.05).Compared with the control group,the serum suPAR levels of the IgAN-FSGS and FSGS groups were significantly increased,the difference was statistically significant(P<0.05).Immunohistochemistry staining showed that uPAR and PAI-1 were mainly expressed in the glomerulus,mainly in the capillary wall and podocytes of the glomerulus,and the strength trend was IgAN-FSGS group>FSGS group>IgAN group>control group,while no significant expression was found in renal tubules and renal interstitial.There was no significant difference in uPAR and PAI-1 expression in the heart tissues of the four groups(P>0.05).Compared with the control group,uPAR expression in brain tissue of IgAN group,FSGS group and IgAN-FSGS group was slightly increased,
