摘　　要：目的 比较感冒清热片和常规用药治疗感冒的临床疗效和经济性。方法 对感冒清热片与荆防合剂或葛根汤颗粒治疗感冒进行Meta分析。建立决策树模型,从医疗系统角度进行成本-效果分析。效果参数为临床有效率来源于临床文献Meta分析,成本参数为药品治疗成本,计算增量成本-效果比(ICER),并通过单因素敏感性分析检验基础分析结果的稳定性。结果 Meta分析结果显示,感冒清热片与荆防合剂相比,治疗感冒的临床疗效更优,差异有统计学意义[RD=0.93,95%CI(0.87,0.99),P<0.000 01];感冒清热片与葛根汤颗粒相比,治疗感冒的临床疗效更优,差异有统计学意义[RD=0.70,95%CI(0.62,0.77),P<0.000 1]。成本-效果分析显示,感冒清热片组与荆防合剂组、葛根汤颗粒相比的ICER为-0.56~0.49、0.001,采用感冒清热片治疗感冒,每多提高1单位临床有效率,患者需多支付-0.56~0.49元、0.001元。结论 感冒清热片治疗感冒的临床疗效优于葛根汤颗粒或荆防合剂治疗,当意愿支付值大于相应的ICER值,感冒清热片比其他两种药品更具经济性。Objective To compare the clinical efficacy and economy of Ganmao Qingre tablets and conventional drugs in the treatment of colds.Methods The meta-analysis of Ganqingre tablets and Jingfang mixture or Gegen Tang granules in the treatment of colds was carried out.A decision tree model is established to carry out cost-effectiveness analysis from the perspective of medical system.Efficacy parameter was clinical effective rate derived from clinical literature meta-analysis,cost parameter was drug treatment cost,incremental cost-effectiveness ratio(ICER)was calculated,and the stability of basic analysis results was tested by single factor sensitivity analysis.Results The results of meta-analysis showed that compared with Jingfang Mixture,Ganqingre tablet had better clinical efficacy in the treatment of colds,and the difference was statistically significant[RD=0.93,95%CI(0.87,0.99),P<0.00001].Compared with Gegen Tang granules,Ganqingre tablets had better clinical efficacy in the treatment of colds,and the difference was statistically significant[RD=0.70,95%CI(0.62,0.77),P<0.0001].Cost-effectiveness analysis showed that the ICER of Ganqingre tablet group was-0.56~0.49 and 0.001 compared with Jingfang mixture group and Gegen Tang granules.When Ganqingre tablet was used to treat colds,patients had to pay-0.56~0.49 and 0.001 yuan more for each additional unit of clinical effective rate.Conclusion The clinical efficacy of Ganqingre tablets in the treatment of colds is better than that of Gegen Tang granules or Jingfang mixture.When the willing payment value is greater than the corresponding ICER value,Ganqingre tablets are more economical than the other two drugs.
