窄谱强脉冲光联合氨甲环酸治疗色素合并血管型黄褐斑的临床研究

摘　　要：目的 探究窄谱强脉冲光联合氨甲环酸治疗色素合并血管型黄褐斑的临床疗效和安全性。方法 选取2021年1月至2024年8月苏州市立医院收治的49例色素合并血管型黄褐斑患者,其中氨甲环酸组10例、窄谱强脉冲光组18例、氨甲环酸联合窄谱强脉冲光组21例。采用黄褐斑面积和严重指数及临床红斑评定量表分析3组患者治疗前后的黄褐斑色素及红斑情况。结果 3组患者治疗后黄褐斑皮损面积和严重度指数较治疗前均降低,差异有统计学意义(P<0.01),氨甲环酸联合窄谱强脉冲光组患者较氨甲环酸组降低显著,差异有统计学意义(P<0.05),较窄谱强脉冲光组降低更显著,差异有统计学意义(P<0.01);窄谱强脉冲光组患者和氨甲环酸联合窄谱强脉冲光组患者治疗后临床红斑评定量表较治疗前均降低,差异有统计学意义(P<0.01),窄谱强脉冲光组患者较氨甲环酸组降低更显著,差异有统计学意义(P<0.05),氨甲环酸联合窄谱强脉冲光组患者较氨甲环酸组降低更显著,差异有统计学意义(P<0.05)。3组患者均未见严重不良反应及复发。结论 窄谱强脉冲光联合氨甲环酸治疗色素合并血管型黄褐斑安全有效,有一定临床应用价值。Objective To explore the clinical efficacy and safety of narrow-spectrum intense pulsed light combined with tranexamic acid in the treatment of pigment combined with vascular melasma.Methods A total of 49 patients with pigment combined with vascular melasma admitted to Suzhou Municipal Hospital from January 2021 to August 2024 were collected,including 10 cases in tranexamic acid group,18 cases in narrow-spectrum intense pulsed light group,and 21 cases in tranexamic acid combined with intense pulsed light group.The melasma area and severity score and clinical erythema rating scale were used to analyze the melasma pigment and erythema before and after treatment in the three groups.Results After treatment,the area and severity index scores of melasma lesions in three groups of patients decreased significantly compared to before treatment(P<0.01).The combination of tranexamic acid and narrow spectrum intense pulsed light group showed a significant decrease compared to the tranexamic acid group,with a statistically significant difference(P<0.05).The decrease was more significant than that in the narrow spectrum intense pulsed light group,with a statistically significant difference(P<0.01);After treatment,the erythema scores of the narrow spectrum intense pulse light group and the combination of tranexamic acid and narrow spectrum intense pulse light group decreased significantly compared to before treatment(P<0.01).The narrow spectrum intense pulse light group showed a more significant decrease than the tranexamic acid group,and the difference was statistically significant(P<0.05).The combination of tranexamic acid and narrow spectrum intense pulse light group showed a more significant decrease than the tranexamic acid group,and the difference was statistically significant(P<0.05).No serious adverse reactions or recurrence cases were observed in all three groups.Conclusions Dye pulsed light combined with tranexamic acid in the treatment of pigment combined with vascular melasma is safe and effective,worthy of clinical
