中药联合免疫治疗对封闭抗体阴性复发性流产患者的疗效分析

摘　　要：目的探究中药联合免疫治疗对封闭抗体阴性复发性流产(RSA)患者的有效性及安全性。方法选取2021年1月至2022年3月石家庄市妇幼保健院生殖医学科就诊的202例封闭抗体阴性RSA患者,按照随机数字表法随机分为免疫组(n=67)、中药组(n=67)和联合组(n=68),免疫组实施淋巴细胞主动免疫治疗,中药组实施中药治疗,联合组实施中药联合免疫治疗,对比3组妊娠情况、封闭抗体情况、炎症因子、免疫细胞水平、妊娠成功率与不良反应情况。结果联合组的妊娠成功率高于免疫组、中药组(P<0.05);治疗后,3组抗CD3-BE、抗CD4-BE、抗CD8-BE水平均升高,且联合组的封闭抗体水平与封闭抗体转阳率高于免疫组、中药组(P<0.05);3组Th17细胞降低,Treg细胞水平升高,且联合组Th17细胞水平低于免疫组、中药组,Treg细胞水平高于免疫组、中药组(P<0.05);3组治疗后白细胞介素-4(IL-4)、IL-10水平均升高且联合组高于免疫组、中药组(P<0.05);3组不良反应发生率比较,差异无统计学意义(P>0.05)。结论封闭抗体阴性RSA患者应用中药联合免疫治疗可以增强免疫功能并促进封闭抗体转阳,有助于减轻炎症反应以及促进妊娠成功,该方案具有较高安全性值得临床推广应用。Objective To investigate the efficacy and safety of traditional Chinese medicine combined immunotherapy in recurrent spontaneous abortion(RSA)patients with blocked antibody negative.Methods A total of 202 RSA patients with blocked antibody negative treated in the Department of Reproductive Medicine of Shijiazhuang Maternal and Child Health Hospital from January 2021 to March 2022 were selected for the study,and divided into immune group(n=67),traditional Chinese medicine group(n=67)and combined group(n=68)according to random number table method.Active lymphocyte immunotherapy was administered in the immune group,while traditional Chinese medicine group received traditional Chinese medicine therapy,the combined group was treated with traditional Chinese medicine and immunotherapy.The pregnancy status,blocking antibody status,inflammatory factors,immune cell level,pregnancy success rate and adverse reactions were compared in the 3 groups.Results The success rate of pregnancy in the combined group was higher than that in the immune group and traditional Chinese medicine group(P<0.05).The levels of anti-CD3-BE,anti-CD4-BE and anti-CD8-BE in the three groups were significantly increased after treatment,and the blocking antibody level and blocking antibody positivity rate in the combined group were higher than those in the immune group and traditional Chinese medicine group;three groups showed a decrease in Th17 cell and an increase in Treg cell the levels,while Th17 cell level in the combined group was lower than the immune group and traditional Chinese medicine group,and Treg cell level was higher than the immune group and traditional Chinese medicine group(P<0.05);the levels of interleukin-4(IL-4)and IL-10 in the three groups increased after treatment,and the combined group was higher than the immune group and traditional Chinese medicine group(P<0.05);there was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion The combination of Chinese medicine and immunother
