摘　　要：目的探讨COOK双球囊联合生化汤剂及中药敷包在瘢痕子宫引产中的应用效果。方法选取2020年1月至2022年1月河北港口集团有限公司港口医院收治的因有医疗指征而行妊娠中期引产的瘢痕子宫患者120例,根据引产方案将其分为对照组(行宫颈COOK双球囊放置术,n=60)和观察组(在对照组基础上联合生化汤剂及中药敷包治疗,n=60),比较两组宫颈Bishop评分、引产时间、产后出血量、恶露持续时间、产后出血率及胎盘胎膜残留率,统计两组并发症发生情况。结果引产后,两组宫颈Bishop评分较引产前均升高(P<0.05),组间比较差异无统计学意义(P>0.05);观察组引产时间、产后出血量、恶露持续时间及发热率、产后出血率及胎盘胎膜残留率均低于对照组,引产总有效率(96.67%)高于对照组(78.33%),差异均有统计学意义(P<0.05)。结论COOK双球囊联合生化汤剂及中药敷包在瘢痕子宫引产中安全有效,能有效减少产后出血,缩短产后恶露持续时间,止痛效果较好,可促进患者产后康复,值得临床推广。Objective To explore the application effect of COOK double-balloon combined with biochemical decoction and traditional Chinese medicine bag in induced labor of scar uterus.Methods From January 2020 to January 2022,120 patients with scar uterus who had induced labor in the second trimester due to medical indications were selected from the Hebei Port Group Co.,Ltd.Port Hospital and they were divided into control group(performing cervical COOK double-balloon placement surgery,n=60)and observation group(combining biochemical decoction and traditional Chinese medicine bag treatment on the basis of the control group,n=60)according to the induced labor scheme.Compared the cervical Bishop score,the time of induced labor,the amount of postpartum hemorrhage,the duration of lochia,the rate of postpartum hemorrhage and the residual rate of placenta and fetal membrane between the two groups,calculated the incidence of complications in the two groups.Results After induced labor,both groups of cervical Bishop scores increased compared to before induced abortion(P<0.05),there was no significant difference between the two groups(P>0.05).The duration of induced labor,postpartum hemorrhage,lochia duration,fever rate,postpartum hemorrhage rate and residual rate of placenta and fetal membrane in the observation group were lower than those in the control group,the total effective rate of induced labor in the observation group(96.67%)was higher than that in the control group(78.33%),and the difference was statistically significant(P<0.05).Conclusion The application of COOK double-balloon combined with biochemical decoction and traditional Chinese medicine bag in induced labor of scar uterus is safe and effective,can effectively reduce postpartum hemorrhage,shorten the duration of postpartum lochia,have a good analgesic effect,and promote the postpartum recovery of patients,which is worthy of clinical promotion.
