胸背部平衡松解术联合子午流注择时穴位按摩治疗产后缺乳的效果分析

摘　　要：目的分析胸背部平衡松解术联合子午流注择时穴位按摩治疗对产后缺乳患者的治疗效果。方法回顾性分析衡水市人民医院产科2020年6月至2022年6月接诊的84例产后缺乳患者,根据治疗方案的不同分为两组,给予低频脉冲电刺激治疗为对照组,在对照组基础上给予胸背部平衡松解术联合子午流注择时穴位按摩治疗的为研究组,两组各42例,比较两组临床疗效、中医症状评分、血清泌乳素(prolactin,PRL)、孕酮(progesterone,P)、雌二醇(estradiol,E_(2))、乳房胀痛程度、泌乳量。结果研究组临床总有效率(95.24%)高于对照组(71.43%),治疗后乳房胀痛、乳汁减少、乳汁稀薄、食欲不振评分均低于对照组(P<0.05)。研究组治疗后血清PRL高于对照组,血清P、E_(2)均低于对照组(P<0.05)。研究组治疗后乳房胀痛程度0级(38.10%)、Ⅰ级率(57.14%)均高于对照组(11.90%、42.86%),Ⅱ级(4.76%)、Ⅲ级率(0)均低于对照组(40.48%、4.76%)(P<0.05)。研究组泌乳充足率(73.81%)高于对照组(23.81%),差异均有统计学意义(P<0.05)。结论胸背部平衡松解术联合子午流注择时穴位按摩可有效增加产后缺乳患者乳汁分泌量,促进PRL释放,调节血清P、E_(2)分泌,缓解乳房胀痛等症状,疗效显著。Objective To analyze the therapeutic effect of chest and back balance release combined with meridian injection and timing acupoint massage on patients with postpartum hypogalactia.Methods A retrospective analysis was conducted on 84 cases of postpartum lactation deficiency patients admitted to the Obstetrics Department of Hengshui People's Hospital from June 2020 to June 2022.They were divided into two groups according to different treatment plans.The control group(42 cases)received low-frequency pulse electrical stimulation treatment,and the study group(42 cases)received chest and back balance release combined with meridian flow injection and timing acupoint treatment on the basis of the control group.The clinical efficacy,traditional Chinese medicine symptom score,serum prolactin(PRL),testosterone(P),estradiol(E_(2)),breast tenderness and milk yield of the two groups were compared.Results The total clinical effective rate of the study group(95.24%)was higher than that of the control group(71.43%).After treatment,the scores of breast tenderness,decreased milk,thin milk and loss of appetite in the study group were lower than those in the control group(P<0.05).The serum PRL of the study group after treatment was higher than that of the control group,and the serum P and E_(2)of the study group after treatment were lower than those of the control group(P<0.05).After treatment,the rates of the degree of breast tenderness grade 0(38.10%)and grade I(57.14%)in the study group were higher than those in the control group(11.90%and 42.86%),and the rates of gradeⅡ(4.76%)and gradeⅢ(0)in the study group were lower than those in the control group(40.48%and 4.76%)(P<0.05).The lactation adequacy rate of the study group(73.81%)was higher than that of the control group(23.81%),and the difference was statistically significant(P<0.05).Conclusion Chest and back balance release therapy combined with meridian flow injection and timing acupoint massage can effectively increase milk secretion,promote PRL release,regulate serum P and
