摘　　要：目的分析某院抗肿瘤药物不合理应用情况及原因,为临床药师医嘱点评及临床合理用药提供参考。方法随机抽取某院2023年全年度包含抗肿瘤药物的病历720份,对其中的抗肿瘤药物进行点评、统计和分析。结果720份病例中,有65份存在不合理用药,占总数的9.03%。主要表现为预处理方案不合理(29.23%)、溶媒不适宜(27.69%)、药品选择不适宜(16.92%)、给药顺序不适宜(10.77%)、适应证不适宜(6.15%)、用法用量不适宜(4.62%)、化疗方案不适宜(3.08%)、配伍禁忌(1.54%)。结论某院抗肿瘤药物存在不合理使用情况。主要表现为预处理方案不合理、溶媒不适宜、药品选择不适宜及给药顺序不适宜等,不合理使用药物主要集中在化疗药物,其次为大分子单抗药物。临床药师应加强对抗肿瘤药的点评并及时干预,促进临床合理用药,保障患者用药安全。Objective To analyze the irrational application of antitumor drugs in a hospital and the reasons for it,so as to provide references for the review of medical prescriptions by clinical pharmacists and the rational use of drugs in clinics.Methods 720 medical records containing antitumor drugs were randomly selected from the hospital for the whole year of 2023,and the antitumor drugs in them were reviewed,counted and analyzed.Results Among the 720 cases,65 had irrational medication,accounting for 9.03%of the total.The main manifestations were irrational pretreatment regimen(29.23%),inappropriate solvent(27.69%),inappropriate drug selection(16.92%),inappropriate order of administration(10.77%),inappropriate indication(6.15%),inappropriate dosage(4.62%),inappropriate chemotherapy regimen(3.08%),and contraindications to compounding(1.54%).Conclusion There was irrational use of antitumor drugs in the hospital.It was mainly manifested as irrational pretreatment program,inappropriate solvent,inappropriate drug selection and inappropriate order of administration,etc.The irrational use of drugs was mainly concentrated in chemotherapeutic drugs,followed by large molecule monoantibody drugs.Clinical pharmacists should strengthen the review of antitumor drugs and intervene in a timely manner to promote the rational use of drugs in the clinic and guarantee the safety of patients'medication.
