无纸化SBAR医护沟通模式在骨科患者预警管理中的应用效果

摘　　要：目的:探讨无纸化现状-背景-评估-建议(SBAR)医护沟通模式在骨科患者预警管理中的应用效果。方法:选取2023年1—6月南方医科大学南方医院增城院区骨科住院患者834例,采用连续抽样法,将2023年1—3月收治的417例患者纳入对照组,2023年4—6月收治的417例患者纳入观察组。对照组医护日常病情汇报采用常规沟通模式,观察组采用无纸化SBAR医护沟通模式。采用沟通能力自评量表、工作能力自评量表、自制满意度调查表比较实施前后护士沟通能力、工作能力、医生对护士满意度,并比较两组患者满意度和护理不良事件、投诉发生率。结果:实施无纸化SBAR医护沟通模式后,护士工作能力自评量表评分、沟通能力自评量表评分高于实施前;医生对护士在报告时机、沟通条理、信息完整、双方默契、护理人员准备方面满意度高于实施前;观察组患者满意度高于对照组,护理不良事件发生率及投诉率低于对照组,差异均有统计学意义(P<0.05)。结论:无纸化SBAR医护沟通模式可提高护士的工作能力和沟通能力,提高医生和患者对护理工作的满意度,减少患者不良事件和投诉的发生。Objective To explore the application effect of paperless situation-background-assessment-recommendation(SBAR)medical communication model in early warning management of orthopedic patients.Methods A total of 834 orthopedic inpatients were selected from January to June 2023 in Zengcheng Hospital,Nanfang Hospital,Southern Medical University,417 patients admitted from January to March 2023 were set as the control group,and 417 patients admitted from April to June 2023 were set as the observation group by continuous sampling method.The routine communication mode was used for daily medical condition reporting in the control group,while the paperless SBAR medical communication mode was used in the experimental group.The Communication Ability Self-rating Scale,Work Ability Self-rating Scale and Self-made Satisfaction Questionnaire were used to compare the nurses'communication ability,work ability and doctors'satisfaction with nurses before and after the implementation,and the patient satisfaction,the incidence of nursing adverse events and the incidence of complaints were compared between the two groups.Results After implementing the paperless SBAR medical model,the nursing staff's Work Ability Self-rating Scale score and Communication Ability Self-rating Scale score were significantly higher than before.Doctors were more satisfied with nurses in terms of reporting timing,communication,information integrity,mutual understanding,and preparation of nursing staff than before implementation.Patient satisfaction of the observation group was higher than that of the control group,and the incidence of adverse events and the incidence of complaints was lower than that of the control group(P<0.05).Conclusions The paperless SBAR medical model can improve the work and communication abilities of nurses,increase the satisfaction of resident doctors and patients with nursing work,and reduce the occurrence of adverse events and complaints in patients.
