观察柴芩温胆汤加减治疗糖尿病性胃轻瘫的临床效果

摘　　要：目的:探讨柴芩温胆汤加减治疗糖尿病性胃轻瘫(Diabetic Gastro Paresis,DGP)的临床效果。方法:将2018年1月―2021年1月泰安市中医医院收治的60例DGP患者纳入研究,采取随机数字表法将患者随机分组,每组30例。对照组采用多潘立酮片治疗,观察组采用柴芩温胆汤加减治疗,比较两组的总有效率、胃肠动力指标、胃排空率、症状评分和不良反应发生率。结果:观察组的总有效率为96.67%(29/30),对照组为80.00%(24/30),观察组高于对照组(χ^(2)=4.043,P<0.05)。治疗后,观察组的胃动素、胃泌素、胃排空率分别为(225.04±19.38)pg/mL、(120.41±12.23)pg/mL和(51.65±10.84)%,对照组分别为(206.27±17.21)pg/mL、(107.92±10.68)pg/mL和(39.58±12.37)%,观察组的胃动素、胃泌素、胃排空率均显著高于对照组(t=3.967、4.213、4.019,P<0.01)。治疗后,观察组的上腹饱胀、反酸嗳气、腹痛评分分别为(2.50±0.83)分、(2.53±0.84)分、(2.44±0.81)分,对照组分别为(3.42±0.97)分、(3.45±0.90)分、(3.35±0.89)分,观察组的各项症状评分均显著低于对照组(t=3.947、4.093、4.142,P<0.01)。观察组的不良反应发生率为0,对照组的不良反应发生率为6.67%,观察组的不良反应发生率低于对照组,但差异无统计学意义(χ^(2)=2.069,P=0.150)。结论:柴芩温胆汤加减治疗可有效改善DGP患者的胃肠动力,促进其胃排空,缓解其症状,疗效显著,且不良反应少,安全性良好。Objective:To investigate the clinical effect of Chaiqin Wendan decoction(柴芩温胆汤)on diabetic gastroparesis(DGP).Methods:60 cases of patients with DGP in our hospital from January 2018 to January 2021 were included in the study and randomly divided into two groups with 30 cases in each group.The control group was treated with Domperidone Tablets,and the observation group was treated with Chaiqin Wendan decoction.The total effective rate,gastrointestinal motility index,gastric emptying rate,symptom score and incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 96.67%(29/30)and that of the control group was 80.00%(24/30),which of the observation group was higher than that of the control group(χ^(2)=4.043,P<0.05).After treatment,the motilin,gastrin and gastric emptying rates of the observation group were(225.04±19.38)pg/mL,(120.41±12.23)pg/mL and(51.65±10.84)%respectively,and those of the control group were(206.27±17.21)pg/mL,(107.92±10.68)PG/mL and(39.58±12.37)%respectively,the motilin,gastrin and gastric emptying rates of the observation group were significantly higher than those of the control group(t=3.967,4.213,4.019,P<0.01).After treatment,the scores of upper abdominal fullness,acid reflux and belching and abdominal pain in the observation group were(2.50±0.83),(2.53±0.84),(2.44±0.81),and those of the control group were(3.42±0.97),(3.45±0.90)and(3.35±0.89)respectively,the scores of various symptoms in the observation group were significantly lower than those in the control group(t=3.947,4.093,4.142,P<0.01).The incidence of adverse reactions in the observation group was 0,and that in the control group was 6.67%,the incidence of adverse reactions in the observation group was lower than that in the control group,but the difference was not statistically significant(χ^(2)=2.069,P=0.150).Conclusion:Chaiqin Wendan decoction can effectively improve gastrointestinal motility,promote gastric emptying and relieve symptoms in
