黑白同治汤联合窄谱中波紫外线治疗白癜风疗效观察

摘　　要：目的:探讨黑白同治汤联合窄谱中波紫外线(Narrow-band Ultraviolet B,NB-UVB)照射治疗白癜风的有效性和安全性。方法:将滨州华海白癜风医院收治的白癜风患者116例按随机数字表法分为治疗组和对照组。治疗组口服黑白同治汤,对照组口服白灵片,两组均联合NB-UVB治疗治疗6个月,并在治疗前及治疗3个月、6个月后分别观察及记录白斑面积与不良反应、复发等情况进行比较和分析。结果:本次研究剔除脱落病例5例,治疗组56例,对照组55例,差异无统计学意义(P>0.05);治疗3个月后,治疗组愈显率75.0%(42/56),总有效率92.9%(52/56);对照组愈显率18.2%(10/55),总有效率38.2%(21/55),差异均具有统计学意义(P<0.05);治疗6个月后,治疗组愈显率89.3%(50/56),总有效率98.2%(55/56),对照组愈显率27.3%(15/55),总有效率61.8%(34/55),差异均具有统计学意义(P<0.05);治疗6个月后,不同类型患者愈显率差异无统计学意义(P>0.05);治疗6个月后,不同部位患者愈显率差异无统计学意义(P>0.05);治疗6个月后,治疗组未出现不良反应,对照组不良反应率为14.5%(8/55),差异具有统计学意义(P<0.05);经6个月随访,治疗组无复发,对照组复发率16.4%(9/55),差异具有统计学意义(P<0.05)。结论:黑白同治汤联合NB-UVB照射对多种证型、不同部位白癜风疗效确切,该技术患者依从度高,无不良反应,巩固后不复发,值得进一步临床推广。Objective:To explore the efficacy and safety of Heibai Tongzhi decoction(黑白同治汤)combined with NB-UVB irradiation in the treatment of vitiligo.Methods:116 patients with vitiligo treated in Binzhou Huahai Vitiligo Hospital were divided into the treatment group and the control group according to the random number table method.The treatment group received oral administration of Heibai Tongzhi decoction and the control group received oral administration of Bailing tablet(白灵片).Both groups were treated with NB-UVB for 6 months,and the white spot area,adverse reactions and recurrence were observed and recorded before treatment,3 months and 6 months after treatment,respectively,for comparison and analysis.Results:In this study,5 cases of shedding were excluded,with 56 cases in the treatment group and 55 cases in the control group,and there was no significant difference(P>0.05);After 3 months of treatment,the cure rate of the treatment group was 75.0%(42/56),and the total effective rate was 92.9%(52/56);the cure rate of the control group was 18.2%(10/55),and the total effective rate was 38.2%(21/55),and the differences were statistically significant(P<0.05);After 6 months of treatment,the cure rate of the treatment group was 89.3%(50/56),and the total effective rate was 98.2%(55/56),and the cure rate of the control group was 27.3%(15/55),and the total effective rate was 61.8%(34/55),and the differences were statistically significant(P<0.05).There was no statistically significant difference in the cure rate among patients of different types after 6 months of treatment(P>0.05);there was no statistically significant difference in the cure rate among patients of different parts after 6 months of treatment(P>0.05);After 6 months of treatment,there was no adverse reaction in the treatment group while the adverse reaction rate in the control group was 14.5%(8/55),and the difference was statistically significant(P<0.05);After 6 months of follow-up,there was no recurrence in the treatment group,while the recurrence r
