耳穴贴压联合针刺水突穴对老年胃肠肿瘤根治术术后睡眠障碍的影响

摘　　要：目的探讨耳穴贴压联合水突穴针刺对老年胃肠肿瘤根治术患者术后睡眠障碍的影响。方法选取2022年12月至2023年12月赣州市人民医院收治的90例老年胃肠肿瘤根治术患者作为研究对象,按照随机数字表法分为对照组和观察组,每组45例。对照组给予耳穴贴压治疗,观察组给予耳穴贴压联合水突穴针刺治疗。比较两组患者睡眠障碍改善总有效率、睡眠质量、疼痛、认知功能、术后恢复时间。结果观察组患者睡眠障碍改善总有效率高于对照组,差异有统计学意义(P<0.05)。术前,两组患者匹兹堡睡眠质量指数(PSQI)总分、简易精神状态检查量表(MMSE)总分比较,差异无统计学意义(P>0.05);术后第1、3、7天,两组患者PSQI总分低于本组术前,MMSE总分高于本组术前,且观察组PSQI总分低于对照组,MMSE总分高于对照组,差异有统计学意义(P<0.05)。观察组术后初次摄食时间、术后首次排便时间、总住院时间短于对照组,差异有统计学意义(P<0.05)。结论耳穴贴压联合水突穴针刺治疗能够进一步提升老年胃肠肿瘤根治术患者术后睡眠质量,减轻患者痛感,改善患者认知功能,促使术后恢复进程加快。Objective To investigate the effect of auricular point pressing combined with acupuncture at Shuitu acupoint on postoperative sleep disorders in elderly patients undergoing radical surgery for gastrointestinal tumors.Methods A total of 90 elderly patients with radical surgery for gastrointestinal tumors admitted to Ganzhou People's Hospital from December 2022 to December 2023 were selected as the research objects,and they were divided into control group and observation group according to random number table method,with 45 cases in each group.The control group was given auricular point pressing treatment,and the observation group was given auricular point pressing combined with acupuncture at Shuitou acupoint treatment.Total response rate,sleep quality,pain,cognitive function and postoperative recovery time were compared between the two groups.Results The total effective rate of sleep disorders improvement in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Before surgery,the total score of Pittsburgh sleep quality index(PSQI)and the total score of mini-mental state examination(MMSE)between the two groups had no statistical significances(P>0.05).On the 1st,3rd and 7th day after surgery,the total scores of PSQI in two groups were lower than those before surgery,the total scores of MMSE in two groups were higher than those before surgery,and the total score of PSQI in observation group was lower than that in control group,the total score of MMSE was higher than that in control group,with statistical significances(P<0.05).The first feeding time,the first defecation time and the total hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Auricular acupoint pressing combined with acupuncture at Shuitu acupoint can further improve sleep quality,relieve pain,improve cognitive function and accelerate postoperative recovery in elderly patients undergoing rad
