超声引导下银质针治疗冈上肌钙化性肌腱炎60例

摘　　要：目的:分析超声引导下银质针治疗冈上肌钙化性肌腱炎的疗效。方法:选取2022年12月至2024年1月收治的60例冈上肌钙化性肌腱炎患者,采用超声引导银质针治疗。对比冈上肌钙化性肌腱炎患者在治疗前、治疗后1周、1个月、3个月肩关节疼痛视觉模拟量表(VAS)评分、Constant-Murley肩功能(CMS)评分、肩关节加州大学洛杉矶分校(UCLA)评分、钙化灶缩小率、美国肩肘外科医生标准化肩部评估表(ASES)评分及临床疗效。结果:与治疗前相比,治疗后1周,冈上肌钙化性肌腱炎患者钙化灶缩小率、ASES评分、肩关节UCLA评分、CMS评分提高,VAS评分降低;治疗后1个月,冈上肌钙化性肌腱炎患者ASES评分、钙化灶缩小率、肩关节UCLA评分、CMS评分上升,VAS评分降低;治疗后3个月,冈上肌钙化性肌腱炎患者钙化灶缩小率、ASES评分、肩关节UCLA评分、CMS评分升高,VAS评分下降,差异均有统计学意义(P<0.05)。冈上肌钙化性肌腱炎患者经超声引导银质针治疗后临床总有效率升高明显,差异有统计学意义(P<0.05)。结论:超声引导下银质针治疗冈上肌钙化性肌腱炎的效果明显,更好地减轻患者疼痛情况,使肩关节功能恢复,促进钙化灶吸收。Objective:To analyze the curative efficacy of silver needle on calcifying tendinitis of supraspinatus muscle under the guidance of ultrasound.Methods:A total of 60 patients with supraspinatus muscle calcifying tendinitis admitted from December 2022 to January 2024 were selected as the study objects.Ultrasound guided silver needle was used for treatment.Visual analogue scale(VAS)score,Constant-Murley shoulder function(CMS)score,University of California Los Angeles(UCLA)score,reduction rate of calcification foci,American shoulder and elbow surgeons standardized shoulder assessment form(ASES)score and clinical efficacy of patients with supraspinatus muscle calcification tendinitis were compared before treatment,1 week,1 month and 3 months after treatment.Results:Compared to before treatment,1 week after treatment,inpatients with supraspinatus muscle calcifying tendinitis,the reduction rate of calcification foci,ASES score,shoulder joint UCLA score and CMS score were increased,while the VAS score was decreased.1 month after treatment,inpatients with supraspinatus muscle calcifying tendinitis,ASES score,reduction rate of calcification foci,shoulder joint UCLA score and CMS score increased,while the VAS score decreased.3 months after treatment,inpatients with supraspinatus muscle calcifying tendinitis,the reduction rate of calcification foci,ASES score,shoulder joint UCLA score and CMS score were higher,and the VAS score was decreased.The difference was statistically significant(P<0.05).The total effective rate of patients with supraspinatus muscle calcification tendinitis increased significantly after ultrasound-guided silver needle treatment.The difference was statistically significant(P<0.05).Conclusion:Silver needle under the guidance of ultrasound in the treatment of supraspinatus muscle calcifying tendinitis is obvious,to better reduce the patient's pain,to restore the function of shoulder joints,promote calcification focus absorption.
