不同时间应用头孢呋辛对闭合性骨折内固定治疗患者术后Ⅰ类切口感染和炎症反应的影响

摘　　要：目的探讨不同时间应用头孢呋辛,对闭合性骨折内固定治疗患者术后Ⅰ类切口感染和炎症反应的影响。方法回顾性选择2018年1月—2023年12月北京积水潭医院贵州医院收治的闭合性骨折内固定患者247例,根据头孢呋辛用药时间不同分为对照组102例和研究组145例。对照组用药时间为术后;研究组用药时间为围手术期,比较两组患者切口愈合及感染情况,并比较外周血炎症因子改善情况。结果研究组切口愈合等级和甲级切口愈合率均优于对照组;而切口愈合时间和缝线拆除时间短于对照组,差异有统计学意义(P<0.05)。研究组切口深度感染比例大于对照组;而感染治疗时间和皮肤愈合时间短于对照组,差异有统计学意义(P<0.05)。研究组外周血急性期蛋白、肿瘤坏死因子-α、白介素-1、白介素-6、白介素-8水平低于对照组,差异有统计学意义(P<0.05)。结论头孢呋辛在围手术期用于闭合性骨折患者的内固定治疗,有助于促进切口愈合和预防感染,并且能降低机体炎症反应。Objective To explore the effect of different durations of application of cefuroxime on postoperative Class Ⅰ incision infection and inflammatory response in patients with closed fractures treated with internal fixation.Methods A total of 247 patients with closed fracture internal fixation admitted to Guizhou Hospital of Beijing Jishuitan Hospital from January 2018 to December 2023 were retrospectively selected.They were divided into a control group of 102 cases and a study group of 145 cases based on the duration of cefuroxime medication.The control group received medication after surgery.The study group used medication during the perioperative period,and compared the wound healing and infection status between the two groups of patients,as well as the improvement of peripheral blood inflammatory factors.Results The wound healing level and Grade A wound healing rate of the research group were better than those of the control group,while the wound healing time and suture removal time were shorter than those in the control group(P<0.05).The proportion of deep incision infections in the research group was higher than that in the control group,while the infection treatment time and skin healing time were shorter than those in the control group(P<0.05).The levels of acute phase protein,tumor necrosis factor-α,interleukin-1,interleukin-6,and interleukin-8 in the peripheral blood of the study group were lower than those of the control group(P<0.05).Conclusion Cefuroxime used for internal fixation treatment in patients with closed fractures during the perioperative period can promote incision healing,prevent infection,and reduce the body's inflammatory response.
