侧孔骨水泥推注器在双侧穿刺经皮椎体成形术中的应用

摘　　要：目的探讨侧孔骨水泥推注器在双侧穿刺经皮椎体成形术(PVP)中的应用效果。方法将57例老年骨质疏松性椎体压缩骨折(OVCF)患者按照骨水泥推注器是否有侧孔分为观察组(采用侧孔骨水泥推注器行双侧穿刺PVP治疗,29例)和对照组(采用常规直型骨水泥推注器行双侧穿刺PVP治疗,28例)。比较两组手术时间、骨水泥推注量、骨水泥弥散及渗漏情况、疼痛VAS评分。结果患者均获得随访,时间6~12个月。手术时间、骨水泥推注量、侧位透视骨水泥弥散面积两组比较差异均无统计学意义(P>0.05);正位透视骨水泥弥散面积观察组大于对照组(P<0.01)。术后1周疼痛VAS评分、骨水泥渗漏率观察组均低于对照组(P<0.05)。结论双侧穿刺PVP治疗OVCF时使用侧孔骨水泥推注器,可均匀向椎体内推注骨水泥,不仅利于骨水泥弥散分布,降低骨水泥渗漏率,而且能减轻患者的早期疼痛。Objective To investigate the application effect of lateral hole bone cement injectorin bilateral puncture percutaneous vertebroplasty(PVP).Methods A total of 57 elderly patients with osteoporotic vertebral compression fractures(OVCFs)were divided into the observation group(29 cases were treated by bilateral puncture PVP with lateral bone cement injector)and the control group(28 cases were treated by bilateral puncture PVP with conventional straight bone cement injector)based on whetherthe bone cement injectorhad lateral holes.The surgical time,volume of bone cement injection,diffusion and leakage of bone cement and pain VAS were compared between two groups.Results All patients were followed up for6~12 months.There were no statistical differences between the two groups in terms of surgical time,volume of bone cement injection,and diffusion area of bone cement through lateral fluoroscopy(P>0.05);the diffusion area of bone cement in the observation group was largerthan that in the control group through anteroposteriorfluoroscopy(P<0.01).At 1 week aftersurgery,the pain VAS and bone cement leakage rate of the observation group were lowerthan those of the control group(P<0.05).Conclusions The lateral hole bone cement injectorin bilateral puncture PVP forOVCF can uniformly push bone cement into the vertebrae,which not only facilitates the distribution of bone cement and reduces the rate of bone cement leakage,but also can relieve early pain of patients.
