摘　　要：目的比较经椎间孔入路腰椎椎间融合术(TLIF)和经肌间隙微创经椎间孔入路腰椎椎间融合术(经肌间隙MIS-TLIF)术后多裂肌影像学指标。方法将24例复发性腰椎间盘突出症患者按治疗方式不同分为观察组(采用经肌间隙MIS-TLIF治疗,14例)和对照组(采用TLIF治疗,10例)。比较两组L_(4-5)节段多裂肌横截面积(CSA)和多裂肌脂肪浸润面积(FLSA)。结果患者均获得随访,时间12~38个月。①L_(4-5)节段多裂肌CSA:两组术后12个月与术前比较差异均无统计学意义(P>0.05);术前、术后12个月两组比较差异均无统计学意义(P>0.05)。②L_(4-5)节段多裂肌FLSA:观察组术后12个月与术前比较差异无统计学意义(P>0.05);对照组术后12个月大于术前(P<0.05);术后12个月观察组小于对照组(P<0.05)。结论与TLIF相比,经肌间隙MIS-TLIF术后多裂肌脂肪浸润率较低。Objective To compare the imaging parameters of multifidus muscle after transforaminal lumbar interbody fusion(TLIF)and minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)via muscle space.Methods The 24 cases with recurrent lumbar disc herniation were divided into the observation group(14 cases were treated with MIS-TLIF via muscle space)and the control group(10 cases were treated with TLIF),according to different treatment methods.The transverse area(CSA)and the fat infiltration area(FLSA)of the multifidus muscle were compared between the two groups.Results All patients were followed up for 12~38 months.①L_(4-5) multifidus muscle CSA:there was no statistical difference in the two groups between 12 months after operation and before operation(P>0.05),and there was no statistical difference between the two groups at 12 months before operation and after operation(P>0.05).②L_(4-5) multifidus muscle FLSA:for the observation group,there was no statistical difference between 12 months after operation and before operation(P>0.05),while it was larger in the control group at 12 months after operation than before operation(P<0.05),the observation group was smaller than the control group at 12 months after operation(P<0.05).Conclusions Compared with TLIF,the rate of multifidus muscle fat infiltration is lower after MIS-TLIF via muscle space.
