摘　　要：目的 探讨闭合复位交叉克氏针内固定治疗儿童桡骨远端Salter-HarrisⅡ型骨折的疗效。方法 采用闭合复位交叉克氏针内固定治疗38例桡骨远端Salter-HarrisⅡ型骨折患儿。记录骨折复位及愈合情况、术后并发症发生情况、腕关节活动度。采用Gartland-Werley评分标准评价腕关节功能。结果 患儿均获得随访,时间3~6个月。骨折均解剖复位。骨折临床愈合时间4~6周,骨性愈合时间3~6个月。患儿均未发生骨折复位丢失、骨骺早闭及腕关节发育畸形等并发症,1例发生指伸肌损伤。末次随访时,腕关节活动度均恢复正常,其中掌屈50°~60°,背伸30°~60°,尺偏30°~40°,桡偏25°~30°;采用Gartland-Werley评分标准评价腕关节功能:优36例,良1例,可1例,优良率97.37%。结论 闭合复位交叉克氏针内固定治疗儿童桡骨远端Salter-HarrisⅡ型骨折,具有创伤小、骨折端固定稳定、患儿可早期功能锻炼、术后并发症少等优点。Objective To investigate the effect of closed reduction and internal fixation with cross Kirschner wire on Salter-Harris typeⅡfracture of distal radius in children.Methods The 38 children with Salter-Harris typeⅡfractures of distal radius were treated by closed reduction cross Kirschner wire internal fixation.Situation of the fracture reduction and healing,postoperative complications and wrist range of motion were recorded.The wrist joint function was evaluated by Gartland-Werley score.Results All the patients were followed up for 3~6 months.All fractures were anatomically reduced.The clinical healing time of fracture was 4~6 weeks,and the bony healing time was 3~6 months.There were no reduction loss,epiphyseal premature closure and deformity of wrist joint development,and 1 case suffered from the finger extensor injury.At the last follow-up,the range of motion of the wrist was restored to normal,of which palmar flexion was 50°~60°,dorsiflexion was 30°~60°,ulnar deviation was 30°~40°and radial deviation was 25°~30°.Gartland-Werley score was used to evaluate the wrist joint function:excellent in 36 cases,good in 1,fair in 1,with and excellent-good rate of 97.37%.Conclusions In the treatment of Salter-Harris typeⅡfracture of distal radius in children,closed reduction cross Kirschner wire internal fixation has the advantages of less trauma,stable fixation of fracture end,early functional exercise and fewer postoperative complications.
