摘 要:目的:了解慢性低度系统炎症与甲状腺结节超声恶性风险分层的关系。方法:研究对象来自2015年参与广州芳村和增城社区甲状腺疾病调查中的1189名甲状腺结节患者。以问卷方式收集有关社会人口学特征和生活方式信息。使用超声检测甲状腺;甲状腺结节应用ACR TI-RADS进行分类。空腹血样被收集以检测血清高敏C-反应蛋白(hs-CRP)。研究对象按hs-CRP三分位水平被分为3组,比较各组甲状腺结节TI-RADS类别的患病率。应用多元线性逐步回归分析hs-CRP水平与甲状腺结节TI-RADS分类的关系。结果:与低hs-CRP水平者比较,高hs-CRP水平者有更高TI-RADS 4-5类甲状腺结节患病率(2.5%vs.24.9%,P<0.05)。线性逐步回归分析显示,以年龄、性别、吸烟、饮酒、体重指数、甲状腺结节数目和大小校正后,hs-CRP水平仍与甲状腺结节TI-RADS分类相关(回归系数0.834,P<0.001)。结论:慢性低度系统炎症与甲状腺结节超声恶性风险分层密切相关。Objective:To explore the relationship between chronic low-grade systemic inflammation and ultrasound malignant risk stratification of thyroid nodules.Methods:The research subjects were 1189 patients with thyroid nodules who participated in the thyroid disorders survey conducted in Guangzhou community in 2015.Information on sociodemographic characteristics and life style was collected using the questionnaires.The thyroid gland was examinated using ultrasound,and the thyroid nodules were classified based on the ACR TI-RADS.Blood samples were collected for the detection of high sensitive C-reactive protein(hs-CRP).The subjects were classified into 3 groups based on the hsCRP tertile levels,and the prevalence of the different TI-RADS categories of thyroid nodules was analyzed among the 3 groups.Linear stepwise regression analysis was used to examine the relationship between hs-CRP and the TI-RADS categories of thyroid nodules.Results:Compared to the subjects in the low hs-CRP group,the subjects in the high hsCRP group had higher prevalence of TI-RADS 4 and 5 category thyroid nodules(2.5%vs 24.9%,P<0.05).Linear stepwise regression analysis showed that hs-CRP levels were associated with the TI-RADS categories of thyroid nodules,even after adjustment for age,sex,smoking,drinking,body mass index,and the numbers and size of thyroid nodules(B:0.834,P<0.001).Conclusion:The chronic low-grade systemic inflammation is associated with the ultrasound malignant risk stratification of thyroid nodules.
