基于共享决策的多元化护理在造血干细胞移植患儿护理中的效果评价

摘　　要：目的探讨基于共享决策的多元化护理在造血干细胞移植患儿护理中的应用效果。方法选择上海市儿童医院2021年6月—2023年6月收治的行造血干细胞移植术72例患儿作为研究对象,随机分为研究组和对照组各36例,对照组采用常规护理,研究组采用常规护理联合基于共享决策的多元化护理,比较两组护理前后焦虑情绪(spence child anxiety scale,SCAS)评分、疼痛情况(face legs activity crying corsolability,FLACC)评分、医疗恐惧感(the child medical fear scale,CMFS)评分和生存质量(pediatric quality of life inventory general core module,PedsQL4.0)评分。结果护理前,两组SCAS评分、FLACC评分、CMFS各维度评分、PedsQL4.0各维度评分进行比较,差异无统计学意义(P>0.05);护理后,两组上述评分均改善,且研究组改善优于对照组,差异有统计学意义(P<0.05)。结论将基于共享决策的多元化护理应用于造血干细胞移植患儿中,可有效缓解焦虑情绪,减轻疼痛,减少医疗恐惧感,提高生存质量。Objective To explore the application effect of diversified care based on shared decision-making in the care of children with hematopoietic stem cell transplantation.Methods Seventy-two children admitted to Shanghai Children’s Hospital for hematopoietic stem cell transplantation from June 2021 to June 2023 were selected as the study subjects,and were randomly divided into the study group and the control group of 36 cases each.The control group adopted conventional care and the study group adopted conventional care combined with diversified care based on shared decision-making.Spence child anxiety scale(SCAS),pain condition(face legs activity crying corsolability,FLACC),medical fear(the child medical fear scale,CMFS)and quality of life(pediatric quality of life inventory general core module,PedsQL4.0)were compared between two groups.Results Before the nursing care,the SCAS score,FLACC score,CMFS scores,and PedsQL4.0 scores showed no significant difference between the two groups(P>0.05).After the nursing care,the above scores of the two groups were improved significantly,and the improvement of the study group was significantly better than that of the control group(P>0.05).Conclusion Applying diversified care based on shared decision-making to children with hematopoietic stem cell transplantation can effectively relieve anxiety,reduce pain,decrease medical fear,and improve the quality of survival.
