县级市综合医院门诊药房处方调配差错原因分析及优化策略探究

摘　　要：目的分析仪征市人民医院门诊药房处方调配差错原因并进行总结,针对以上情况进行相关改进措施的探讨,以达到降低门诊药房总体调配差错率,提高药学综合服务质量的目的。方法收集仪征市人民医院门诊药房2021—2023年门诊药房总处方量及差错处方量并计算差错率,分析差错成因并阐述相关改进措施。结果2021年全年处方量为253605张,差错率为0.016‰,2022年全年处方量为320418张,差错率为0.009‰,2023年全年处方量为364400张,差错率为0.127‰。错误原因主要包括人为因素、环境因素、管理因素、硬件设备等方面。结论处方调配错误时常发生,归根究底主要受人为、环境、管理及设备等方面影响,在门诊药房后续工作中,应大力加强人员管理与培训,定期进行药品维护、检修设备,提高工作人员的服务意识及责任心,保证患者用药合理安全。Objective To analyze the reasons for prescription dispensing errors in outpatient pharmacy of the People’s Hospital of Yizheng and summarize.Discuss relevant improvement measures based on the above situation,in order to reduce the overall dispensing error rate of outpatient pharmacies and improve the quality of pharmaceutical comprehensive services.Methods The total prescriptions and wrong prescriptions of outpatient pharmacy in the People’s Hospital of Yizheng from 2021 to 2023 were collected and the error rate was calculated.The causes of errors were analyzed and the relevant improvement measures were elaborated.Results The total number of prescriptions in 2021 was 253,605,with an error rate of 0.016‰.In 2022,the total number of prescriptions was 320418,with an error rate of 0.009‰.In 2023,the total number of prescriptions was 364400,with an error rate of 0.127‰.The main reasons for errors included human factors,environmental factors,management factors,hardware equipment,and other aspects.Conclusion Prescription dispensing errors often occur,and ultimately are mainly affected by factors such as human factors,environment,management,and equipment.In the follow-up work of outpatient pharmacies,efforts should be made to strengthen personnel management and training,regularly maintain drugs and inspect equipment,improve the service awareness and sense of responsibility of staff,and ensure the rational and s afe use of medication by patients.
