基于代谢组学联合转录组学探讨中西医结合四联疗法治疗带状疱疹后神经痛的作用机制

摘　　要：带状疱疹后神经痛的发病机理与“虚气留滞”理论高度契合。带状疱疹后神经痛是带状疱疹的后病理时期,其发病离不开“虚”与“瘀”,此类患者多年老体弱,正气亏虚,不能托邪外出,而致邪毒潜伏人体,日久五脏虚损,脉络瘀阻。“虚气”“留滞”互为因果,是对带状疱疹后神经痛中医病因病机的高度概括。代谢组学是沟通小分子代谢物与机体生理、病理反应的重要桥梁,对于寻找带状疱疹后神经痛的生物标记物具有重大意义。转录组学是串联基因水平信息与机体生理、病理状态下生命现象的重要纽带,对于揭示带状疱疹后神经痛的发病机制具有重大意义。转录组学联合代谢组学可阐释疾病发展过程中机体的动态变化水平,有利于进一步揭示带状疱疹后神经痛“虚气留滞”的现代化科学内涵,准确地反映中西医结合四联疗法治疗带状疱疹后神经痛的临床疗效。The pathogenesis of postherpetic neuralgia is highly consistent with the theory of"Qi deficiency and stagnation".Postherpetic neuralgia is the post-pathological period of herpes zoster,and its onset is inseparable from"deficiency"and"stasis".Such patients are old and frail for many years,whose Zheng Qi is deficient,and they cannot dispel the evil factors out,resulting in evil factor induced toxin lurking in the body,and the Five Zang organs are weakened for a long time,with meridians being blocked."Qi deficiency"and"Qi stagnation"are mutually causal,which is a high-level summary of the etiology and pathogenesis of postherpetic neuralgia in traditional Chinese medicine.The metabolome is an important bridge to communicate small molecule metabolites with the physiological and pathological responses of the body,which is of great significance for the search for biomarkers of postherpetic neuralgia.Transcriptomics is an important link between the tandem gene-level information and the physiological and pathological life phenomena of the body,which is of great significance for revealing the pathogenesis of postherpetic neuralgia.Transcriptome combined with metabolomics can explain the dynamic changes of the body during the development of the disease,which is conducive to further revealing the modern scientific connotation of"Qi deficiency stagnation"in postherpetic neuralgia,and accurately reflecting the clinical efficacy of quadruple therapy combined with integrative medicine in treatment of postherpetic neuralgia.
