探讨八髎穴烧山火针刺联合补中益气汤治疗脾肾虚型轻、中度女性压力性尿失禁的临床疗效

摘　　要：目的:探讨八髎穴烧山火针刺联合补中益气汤治疗脾肾虚型轻、中度女性压力性尿失禁的临床疗效。方法:按随机数字表法将2021年1月-2023年12月重庆市中医院妇科收治的60例压力性尿失禁患者分为对照组和治疗组,每组30例。对照组予以补中益气汤加减治疗,治疗组加用八髎穴烧山火针刺法,两组患者均连续治疗8周。比较两组临床疗效、尿失禁评分、临床相关指标和生活质量、安全性。结果:治疗后,治疗组总有效率高于对照组,总漏尿次数、总排尿次数均少于对照组,总排尿量大于对照组,综合评分低于对照组,差异均有统计学意义(均P<0.05)。治疗后,治疗组1 h尿垫试验漏尿量少于对照组,膀胱颈移动度、膀胱尿道后角均小于对照组,盆膈裂孔左右径、前后径及面积均小于对照组,差异均有统计学意义(均P<0.05)。治疗后,治疗组生活质量测定量表(Quality of Life-Brief Scale,QOL-BREF)各项评分均高于对照组,差异有统计学意义(P<0.05)。两组治疗期间均未见明显不良反应。结论:脾肾虚型轻、中度女性压力性尿失禁患者采用八髎穴烧山火针刺联合补中益气汤治疗疗效显著,有助于增强患者盆底肌力,减轻尿失禁各临床症状,提高患者生活质量,对促进患者预后恢复具有重要作用,值得推广应用。Objective:To investigate the clinical efficacy of the Shaoshanhuo acupuncture(烧山火针刺)at Baliao points(八髎穴)plus Buzhong Yiqi decoction(补中益气汤)in the treatment of mild and moderate female stress urinary incontinence with deficiency of spleen and kidney.Methods:According to the random number table method,60 patients with stress urinary incontinence admitted to the department of gynecology in Chongqing hospital of traditional Chinese medicine from January 2021 to December 2023 were divided into control group and treatment group,30 cases in each group.The control group was treated with Buzhong Yiqi decoction.The treatment group was additionally treated with the Shaoshanhuo acupuncture at Baliao points.Both groups were treated for 8 weeks.The clinical efficacy,urinary incontinence score,clinical related indicators,quality of life and safety were compared between the two groups.Results:The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the total times of urinary leakage and urination in the treatment group were less than those in the control group.The total urine output was greater than that of the control group.The comprehensive score was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the amount of urine leakage in the treatment group was less than that in the control group in the 1H urine pad test.The mobility of bladder neck and the posterior angle of bladder and urethra in the control group were less than those in the control group.The left-right diameter,anterior posterior diameter and area of the pelvic diaphragm hiatus were smaller than those of the control group.The difference was statistically significant(P<0.05).After treatment,the scores of QOL-BREF in the treatment group were higher than those in the control group,and the difference was statistically significant(P<0.05).No obvious adverse reactions were observed
